#### Date

**Key:** date

**Type:** date

**Default:** 2026-10-20

#### Background

**Key:** background

**Type:** textarea

**Default:** Packer 2 is the constraint for the retail line. Over the last eight weeks it has missed its weekly plan five times, mostly because of short stops and slow running.

#### Current condition

**Key:** current

**Type:** textarea

**Default:** Availability 82 percent, performance 71 percent, quality 98 percent, OEE 57 percent. Micro stops account for about 90 minutes per shift.

#### Goal

**Key:** goal

**Type:** textarea

**Default:** Raise OEE from 57 percent to 68 percent within 12 weeks by cutting micro stops by half and restoring the standard rate.

#### Root cause analysis

**Key:** rootcause

**Type:** textarea

**Default:** 5 Whys: film jam at the wrapper, worn feed rollers, no spare in the cell, no preventive replacement interval, interval never set after the last model change.

#### Countermeasures

**Key:** countermeasures

**Type:** textarea

**Default:** 1. Replace rollers and set a 500 hour interval. 2. Stock one spare set in the cell. 3. Add sensor check to start-of-shift list. 4. Re-time standard cycle.

#### Follow-up and results

**Key:** followup

**Type:** textarea

**Default:** Review micro stop log weekly. Confirm OEE trend at week 4, 8 and 12. Standardize on the other packers if results hold.

**Notes:** This template follows the common A3 storyline used in lean practice. Adapt headings to your company's own format. Keep each box short enough that the report can be read in five minutes.

## Content

**Intro:** An A3 report forces a team to write the problem, the data, the cause and the actions on a single page. This template gives you the standard storyline so you can spend your time on the thinking instead of the layout.

**Sections:**

#### How to use the template

**Body:** Fill in the title, owner and sponsor, then work the boxes from left to right: background, current condition, goal, root cause, countermeasures, plan and follow-up. Write facts with numbers where possible, and keep each box to a few lines. Export to PDF for print or to Word if the team wants to edit offline. The implementation table is left blank on purpose so you can fill actions in on the shop floor with the people who will do the work.

#### The storyline behind A3

**Body:** The A3 is named for the paper size and became a lean tool through the Toyota practice of plan, do, check, act on one page. The left side describes the problem and the analysis, the right side the solution and the follow-up. The format works because it forces a clear causal chain: each countermeasure should address a stated root cause, and each root cause should explain the gap in the current condition.

**Bullets:**

- Background: why this matters to the business
- Current condition: measured, not opinion
- Goal: specific, with a date
- Root cause: verified, not assumed
- Countermeasures: tied to causes, with owners

#### Using A3 for OEE problems

**Body:** For an OEE loss, start with the loss Pareto to prove that the problem is among the biggest. State availability, performance and quality separately in the current condition, so the goal targets one factor. Verify the root cause by observation at the machine, using the five whys until you reach something that a process or system change can prevent. Close the A3 only after the follow-up shows the loss has actually fallen.

#### Common mistakes

**Body:** The most frequent error is jumping to a solution before the cause is verified, so the countermeasure box is full and the root cause box is thin. Another is writing the report alone at a desk. An A3 is a conversation, and the sponsor or coach should review it with the owner at the process. Avoid vague goals like improve reliability, and avoid actions without owners or dates, which are wishes rather than plans.

**Cta:** If the data for your current condition lives in an ERP, we can help you build a view that fills these numbers automatically for the next A3.

**Faq:**

#### Does an A3 have to fit on one page?

**Answer:** That is the discipline. The name comes from the paper size, but the real rule is brevity. If you cannot summarize the problem and plan concisely, the thinking is probably not finished yet.

#### Who should own the A3?

**Answer:** The person closest to the problem, usually a supervisor or engineer, with a sponsor or coach who reviews and challenges. Ownership by someone far from the process tends to produce a report rather than a fix.

#### How is an A3 different from an 8D?

**Answer:** Both are structured problem solving. An 8D is usually a customer or supplier complaint response with containment steps, while an A3 is a broader learning and improvement format. Many plants use A3 for chronic internal losses like OEE.

#### Can I export it to Word?

**Answer:** Yes. Fill in the fields and export to PDF or Word. The content stays in your browser and is not sent to a server.
