**Notes:** Align response deadlines with your supplier quality agreement. Escape point analysis (why the problem was not caught at the supplier) is expected by most primes. This is a starting template and does not replace contractual terms.

## Content

**Intro:** A SCAR gets a supplier to contain a problem, find the real cause and fix it before the next shipment. This template sets out clear response steps and dates so your request is easy to answer and easy to audit.

**Sections:**

#### How to use the SCAR template

**Body:** Fill the identification fields, describe the defect with measured values against the requirement, and set realistic response dates. Export to PDF or Word and send it with supporting photos and inspection data. When the supplier responds, review the root cause critically and do not close the request until effectiveness is verified on later shipments.

**Bullets:**

- Attach objective evidence and photographs
- Give containment due dates in days, not weeks
- Ask for escape point analysis
- Record the closure and verification date

#### What a strong response contains

**Body:** Good responses separate three causes: why the defect occurred, why it was not detected before shipment, and which system allowed both. Corrective actions should address each cause with an owner and a date. A response that says operator retrained is usually weak because it treats the symptom and relies on memory.

#### Severity and timing

**Body:** Critical issues that affect safety or function normally need containment within a day or two and immediate notification of other customers' risk where relevant. Major issues may allow a week or two. Minor issues can be handled in the normal cycle. Document your thresholds in the supplier quality agreement so both sides know the clock.

#### Common mistakes

**Body:** Common problems include accepting a generic root cause, closing on promise rather than evidence, skipping effectiveness checks, and not feeding the result back into the supplier's rating. Link the SCAR to the NCR, record the cost of the escape, and review open SCARs in management review.

**Cta:** Netray can help connect SCARs to receiving inspection and supplier rating data in your ERP so repeat issues surface early.

**Faq:**

#### Is a SCAR the same as an 8D?

**Answer:** They overlap. An 8D is a structured problem solving method, while a SCAR is the request issued to the supplier. Many companies ask the supplier to answer with an 8D format. This template uses fewer steps but covers the same ground.

#### When should I issue a SCAR?

**Answer:** Issue one for repeated or significant nonconformances, delivery of product that escaped the supplier's process controls, or any issue affecting safety or function. Define your triggers in the supplier procedure.

#### What does AS9100 say about corrective action?

**Answer:** Clause 10.2 requires you to react to nonconformities, evaluate the need for action to eliminate causes, implement it and review effectiveness. Keep records of the nature of the nonconformity, the action taken and the results.

#### How do I verify effectiveness?

**Answer:** Review the next several lots or a defined period of shipments for recurrence, and audit the changed process step if needed. Record the evidence before closing.
