## Short answer

A pre-closure checklist that confirms the corrective action is complete, effective, documented and ready for audit review.

## Category

quality

## Industry

Discrete Manufacturing

## Audience

- Quality manager
- Quality auditor
- CAPA owner

## Tool

**Kind:** checklist

**Groups:**

#### Problem definition and containment

**Items:**

**Text:** Problem statement is specific, with part, quantity, location and detection point

**Critical:** true

Suspect population defined by lot, date code or serial range

**Text:** Containment actions recorded with quantities and dates

**Critical:** true

Customer or affected downstream process notified where required

Stock on hold released only with documented criteria

Related parts, lines and suppliers checked for the same risk

#### Root cause

**Items:**

Root cause method documented (5 whys, fishbone, fault tree or similar)

**Text:** Root cause explains why the defect occurred

**Critical:** true

**Text:** Root cause explains why the defect escaped detection

**Critical:** true

Evidence supports the cause: data, test, sample or reproduction

Systemic cause identified beyond operator error

Other possible causes ruled out and recorded

#### Corrective and preventive action

**Items:**

**Text:** Action addresses the verified root cause directly

**Critical:** true

Each action has a named owner and completed date

Procedures, work instructions and routings updated and revision controlled

Error-proofing or control considered before relying on training alone

FMEA and control plan reviewed and updated where applicable

Changes validated before release to production

Preventive action extended to similar processes and products

#### Effectiveness verification

**Items:**

**Text:** Metric and pass criteria defined before the check

**Critical:** true

Monitoring period long enough for the defect to recur if still present

Data from enough production volume to be meaningful

**Text:** Result shows no recurrence or improved capability

**Critical:** true

Verification done by someone independent of the owner

Floor check confirms the new method is actually in use

#### Records and communication

**Items:**

Evidence attached: reports, photos, data, updated documents

NCR, complaint and CAPA records cross-referenced

**Text:** Customer response or 8D accepted and closed by the customer where required

**Critical:** true

Lessons learned shared with relevant teams

Quality manager approval signed and dated

Record retained for the period your system and contract require

**Scoring Note:** Every critical item must be checked before closing. A CAPA with all critical items checked and most others complete is ready. If a critical item is open, keep the CAPA open or extend the monitoring.

## Content

**Intro:** Closing a CAPA too early is one of the most common audit findings in ISO 9001, AS9100 and IATF 16949 environments. This checklist walks through the five stages so you can confirm the record is complete and the fix really worked.

**Sections:**

#### How to use the checklist

**Body:** Go through the groups in order with the CAPA file open. Tick each item only when you can point to the evidence in the record. Items marked critical are the ones that most often decide whether an auditor or customer accepts the closure: a specific problem statement, containment, a cause that explains both occurrence and escape, an action aimed at the cause, a defined effectiveness test and the customer's acceptance. Export the completed list and attach it to the CAPA as a closure record.

**Bullets:**

- Tick only what the record can prove
- Treat critical items as mandatory
- Have someone other than the owner run the checklist
- Attach the completed list to the CAPA

#### Occurrence and escape

**Body:** A strong root cause answers two questions: why did the defect happen, and why was it not detected before it reached the customer or the next process. Many CAPAs address only the first. If the escape cause is missing, the same detection gap remains open for the next defect. In an 8D, this corresponds to separating the occurrence and escape root causes, and then defining a permanent corrective action for each.

#### Reading the result

**Body:** If every critical item is checked and the rest are mostly complete, close the CAPA. If one or two non-critical items are open, close with a documented plan for the remainder. If a critical item is missing, the record is not ready. Typical gaps are no independent verification, no check that the changed procedure is used on the floor, and an effectiveness window shorter than the production cycle of the part.

#### Common mistakes

**Body:** Retraining as the sole action, closing on implementation date, skipping the customer's acceptance, and leaving the FMEA or control plan unchanged are the repeat offenders. A related issue is evidence stored in email instead of the record. In your ERP or quality system, make the closure checklist a required attachment so a CAPA cannot move to closed without it.

**Cta:** Netray can add a closure gate to your ERP or quality workflow that blocks closing a CAPA until the evidence is attached.

**Faq:**

#### Which items are mandatory before closing a CAPA?

**Answer:** The ones marked critical here: a specific problem, containment, a cause that explains occurrence and escape, an action on that cause, a defined effectiveness test with a positive result, and customer acceptance where required.

#### Can I close a CAPA while effectiveness is still being monitored?

**Answer:** Most quality systems keep it open, or in a verification status, until the check passes. Closing earlier with a pending check is a frequent audit finding.

#### Who should review a CAPA before closure?

**Answer:** Someone independent of the CAPA owner, normally the quality manager or a quality engineer, and the customer when their requirements call for acceptance.

#### Does this checklist replace my procedure?

**Answer:** No. It is a working aid. Follow your own CAPA procedure and contract requirements, and adjust the list to match them.
