## Content

**Intro:** When a batch goes outside its limits, the paperwork decides whether the plant learns anything. This template guides the investigation from the first observation through containment, product impact, root cause and effectiveness checks.

**Sections:**

#### How to use it

**Body:** Open the report as soon as the deviation is noticed and record facts before memory fades. Fill the event description with what was observed, not conclusions. List containment actions with times. Only then assess product impact and root cause. Export to PDF or Word and route for approval. The action tracking table should have names and dates, otherwise the corrective actions will drift.

#### Investigating the root cause

**Body:** Stop at a cause that, if removed, would prevent recurrence. 'Operator error' is rarely a root cause on its own. Ask why the system allowed the error: missing checklist step, unclear label, bad instrument layout or schedule pressure.

**Bullets:**

- Collect process data, alarms, logs and interviews
- Compare with the last good batch
- Check equipment status, calibration and maintenance history
- Use 5 Whys or a fishbone to reach a system cause
- Confirm the cause with evidence before closing

#### Assessing product impact

**Body:** Decide what the deviation could have affected: quality attributes, safety, labelling, stability or traceability. Test where evidence is needed rather than guessing. Check upstream and downstream: other batches made with the same equipment, same raw material lot or same shift. Document the reasoning even when the conclusion is no impact, because auditors look for that.

#### Common mistakes

**Body:** Common problems are closing with retraining as the only action, no effectiveness check, vague due dates, ignoring other batches, and delaying the report until the batch is already shipped. Link each deviation to the batch record and review open deviations weekly.

**Cta:** Netray can help pull batch, equipment and inspection history from your ERP into the investigation so the data gathering takes minutes, not days.

**Faq:**

#### How do I classify a deviation as minor, major or critical?

**Answer:** Base it on potential impact on safety, product quality and compliance. Critical means likely harm or a regulatory breach, major means possible quality impact, minor means no impact on quality. Define the criteria in your procedure.

#### How quickly must a deviation be investigated?

**Answer:** Your procedure sets the timeline, often a few days for containment and 30 days or so for closure. Critical issues need immediate action. Missing the timeline should itself be escalated.

#### What is an effectiveness check?

**Answer:** A planned follow-up after implementing actions to show the problem did not recur. For example, review the next ten batches for the same event and record the result.

#### Is a deviation the same as a nonconformance?

**Answer:** They overlap. A deviation is a departure from an approved process or procedure, while a nonconformance is a failure to meet a requirement. Many plants use one form for both.
