## Content

**Intro:** Closing a corrective action on the day the fix is implemented proves nothing. Auditors under ISO 9001 and AS9100 look for evidence that the action actually worked. This template records the metric, the monitoring window and the pass criteria, then the data and the decision.

**Sections:**

#### How to use the template

**Body:** Fill the metric, the monitoring period and the pass criteria when you approve the corrective action, so the test is fixed before any data arrives. After the monitoring period, record the results with real numbers and tick the evidence you reviewed. The decision field has three outcomes. Effective closes the record. Not effective reopens the root cause analysis. Extend monitoring applies when volume was too low to judge, for instance only one production lot ran in the window.

**Bullets:**

- Set pass criteria before implementation
- Use a measurable metric, not a feeling
- Check on the floor that the new method is used
- Record the lot numbers the data came from

#### Choosing a metric and period

**Body:** Match the metric to the failure. For a dimensional defect, use process capability such as Cpk, or defect counts per lot. For a delivery or documentation failure, use recurrence count over a set number of orders. The period should span enough production to give the defect a fair chance to return: three lots or 90 days are common, but a low volume part may need six months. A result of zero recurrence over a short or empty period is not evidence, so state the volume produced.

#### Worked example

**Body:** A worn boring bar caused oversize bores. The action sets a 250-part tool life limit in the control and the ERP routing. The verification plan requires a Cpk of at least 1.67 over three consecutive lots and no repeat complaints for 90 days. The results show Cpk values of 1.82, 1.79 and 1.88, with no complaints. The decision is effective, and the CAPA is closed with the SPC export attached.

#### Common mistakes

**Body:** The usual faults are checking too early, using retraining as the only action and as the only evidence, and verifying only that the action was done rather than that it worked. Implementation is not effectiveness. Another fault is a metric the process cannot measure, such as a defect count without a defined inspection. Keep the SPC export or inspection report as an attachment and store the CAPA number against the lots reviewed.

**Cta:** Netray can pull the inspection and complaint history for a part from your ERP into a single effectiveness view, read-only.

**Faq:**

#### What is the difference between verification and validation of a corrective action?

**Answer:** Verification confirms the action was implemented and does not cause new problems. Validation or effectiveness confirms the action actually prevents recurrence. Many quality systems require both before closure.

#### How long should I monitor before closing a CAPA?

**Answer:** It depends on volume and risk. Three production lots or 90 days is a common starting point. Low volume or high risk products often need longer monitoring.

#### What if there is no recurrence but also no production?

**Answer:** That is not evidence of effectiveness. Extend the monitoring until enough units have been produced to give a fair test, and say so in the record.

#### Who should do the effectiveness check?

**Answer:** Ideally someone other than the CAPA owner, such as a quality engineer or manager, so the check is independent of the person who implemented the action.
