## Content

**Intro:** A risk register is the one document a steering committee will actually read when an ERP project wobbles. This template scores each risk by probability times impact, keeps an owner and mitigation next to it, and exports to Excel for your PMO.

**Sections:**

#### How to use the register

**Body:** Add one row per risk, not per worry. Write the description as a cause and effect statement, such as legacy duplicates causing failed item loads. Score probability and impact from 1 to 5, and the score column multiplies them for you. Assign a single named owner, not a team. Set a next review date so rows do not go stale. Export to Excel and paste into your steering pack, or keep it as the live copy for your weekly project meeting.

**Bullets:**

- One owner per risk
- Review high scores weekly
- Close risks explicitly
- Keep occurred risks as issues

#### The scoring method

**Body:** Probability times impact on a 5 by 5 grid is the standard qualitative method in PMBOK style risk management. It is not a precise forecast. Its job is to rank risks so the team spends mitigation effort where it matters. Agree the meaning of each impact level up front, for example 5 means more than four weeks of slip or more than ten percent of budget. Without shared definitions, two people score the same risk differently and the register loses credibility.

**Bullets:**

- Define impact levels in days and money
- Re-score after mitigation
- Keep a residual score column if your PMO wants one

#### Typical ERP risk categories

**Body:** Data migration and integration risks cause most go-live slips: dirty master data, unmapped fields, interfaces to MES, EDI, tax engines and banks. People risks come next, mainly key users who are not released from their day job. Scope creep through late report and customization requests is the third pattern. Vendor risks include consultant turnover and unclear responsibility between the software vendor and the integrator. Seed your register with these and add your own site specific items.

**Bullets:**

- Master data quality
- Interface readiness
- Key user availability
- Customization creep
- Consultant continuity

#### Common mistakes

**Body:** The most frequent mistake is a register full of generic items that never change, such as lack of user adoption. Another is scoring everything 3 by 3 so nothing stands out. A third is never closing risks, which hides real progress. Finally, teams treat the register as a PMO artifact instead of a working tool. If the owner column is blank or the review dates are in the past, the register is decoration. Keep it short, specific and current.

**Cta:** If your risk register keeps surfacing data quality and reporting questions, we can show how read-only AI over your ERP data helps answer them faster. No commitment, and the template is yours either way.

**Faq:**

#### What is a risk register in an ERP project?

**Answer:** It is a living list of things that might go wrong, each with a likelihood, an impact, an owner and a planned response. In ERP projects it is reviewed at the weekly project meeting and summarized for the steering committee.

#### How is the risk score calculated?

**Answer:** Probability from 1 to 5 multiplied by impact from 1 to 5, giving a score from 1 to 25. Higher scores are discussed first. The scale is relative, so agree what each level means before scoring.

#### What is the difference between a risk and an issue?

**Answer:** A risk may happen in the future. An issue has already happened and needs action now. When a risk occurs, change its status to Occurred and log the follow-up work in your issue or action log.

#### How often should we update the register?

**Answer:** Weekly for active risks and at every phase gate. Review high scoring items at each steering committee. Rows with a past review date should be treated as a warning sign.
