## Content

**Intro:** A fiber panel with no map becomes a guessing game the first time a link fails. This worksheet keeps every port, strand pair and far end in one editable table you can export to Excel.

**Sections:**

#### How to use the worksheet

**Body:** Add one row per port or per duplex pair. Enter the panel name, port number, the strand pair inside the trunk or cable, and where the other end lands. Pick the connector type and polish, then set polarity and status. Export the sheet to Excel or PDF and tape a printed copy inside the rack door as a backup to your asset database.

**Bullets:**

- One row per duplex pair, not per single fiber, for LC and SC links
- Use the same panel naming as your DCIM or CMDB
- Mark spare and dark ports so nobody patches into a reserved strand

#### Polarity and polish

**Body:** Polarity errors are the most common cause of a new link not coming up. In a duplex system the transmit on one end must reach receive on the other, which means an odd number of crossovers in the full channel. For MPO trunking, polarity method A, B or C describes how the trunk and cassettes cross fibers. Polish matters too: APC connectors are green by convention and must never be mated to UPC, as the angled end face and flat end face do not mate correctly and cause high loss.

#### Keeping the map accurate

**Body:** A port map is only valuable if it is updated at every move, add or change. Tie updates to your change ticket, and re-export after each batch of work. During audits, sample a few ports with a visual fault locator or a power meter to confirm the map matches reality. Record the date of the last verification in your own document control system.

#### Common mistakes

**Body:** Typical errors are labelling the front of the panel but not the rear, using different names on the panel and in the spreadsheet, and recording only the near end. Always record the far end including the far panel and port. Do not reuse a strand pair that is marked Reserved without updating status first, and never assume a spare strand is dark until verified.

**Cta:** If port and strand data should flow into your ERP or CMDB automatically, we can help design the import and the approval step.

**Faq:**

#### Does one row mean one fiber or one port?

**Answer:** For duplex connectors like LC and SC, use one row per duplex port and list the strand pair. For simplex or MPO connectors, one row per connector with the trunk or fiber range in the strand column works well.

#### Why record polish type?

**Answer:** UPC and APC connectors cannot be mated together without high loss and possible end face damage. Recording polish for each port prevents mismatched patch cords from being used during later moves.

#### How do I show a cross connect across two panels?

**Answer:** Add a row on each panel and write the other panel and port in the far end column. That gives you a trace in both directions from either row.

#### Can I import this into a CMDB?

**Answer:** Export to Excel or CSV and map the columns to your CMDB fields. Most tools accept panel, port and far end as the minimum set for a cable connection record.
