## Content

**Intro:** A splice closure holds hundreds of spliced fibers that nobody will see again until a fault. This record captures what went in, how it was sealed and who did it, so the next crew can work from facts.

**Sections:**

#### How to use the record

**Body:** Fill the header fields on site or right after leaving the handhole, then complete the tray and splice map table by hand or after export. One record per closure, and file it against the closure ID in your GIS or asset system. Keep the sealing method and closure type, because the reopen procedure differs for gel, heat shrink and gasket designs.

**Bullets:**

- Use the same closure ID as the label on the closure
- Record cable IDs exactly as printed on the jacket
- Note any tubes left unspliced and where the slack is stored

#### What the splice map should show

**Body:** The map ties each incoming fiber to its outgoing fiber, tray by tray. Record the splicer's estimated loss for each splice as a quick screen, but treat it as an estimate. Fusion splicers estimate loss from the image of the fibers and the quality of the alignment, and the figure can differ from an OTDR reading. Typical single mode fusion splices land well under 0.1 dB when the cleave and arc are good, so any value much above that deserves a remake.

#### Closure checks that matter

**Body:** Most closure failures are water ingress or strength member slippage, not bad splices. Confirm that strength members are clamped, that bonding or grounding is done where the cable has metallic elements, and that the seal was tested according to the closure maker's instructions. Photograph the open tray stack before closing and store the photos with the record.

#### Common mistakes

**Body:** Leaving the fiber-in and fiber-out columns blank is the usual failure, since the record then proves nothing. Others are reusing a gel seal that was already compressed, mixing tube and fiber numbering schemes between the cable and the tray, and signing off before the OTDR trace is reviewed. Write the color code convention you used in the remarks so a later crew can decode it.

**Cta:** If closure and splice records live in spreadsheets today, we can help bring them into your ERP or asset system so field data and inventory stay linked.

**Faq:**

#### Is one record needed per closure or per tray?

**Answer:** One record per closure is the norm, with the tray and splice map table carrying the per-tray detail. If a closure has many trays, export several copies of the table or continue on a second page and number the pages.

#### Should I record the splicer's loss estimate?

**Answer:** Yes, as a quick quality screen, but label it as an estimate. Acceptance is normally based on OTDR traces taken from both directions, because a single direction can hide gainers and misread splice loss.

#### Can I use this for mass fusion (ribbon) splices?

**Answer:** Yes. Change the Method column to Mass fusion and list the ribbon or fiber group in the fiber-in and fiber-out columns. Record the loss per fiber if your splicer reports it, or per ribbon otherwise.

#### Does this replace my company's quality form?

**Answer:** No. It is a clean starting point you can adapt. If your contract or customer requires a specific form, match their field names and keep this record as backup detail.
