#### Approvals

**Signatures:**

- Requester

## Content

**Intro:** In a plant, an unplanned IT change can stop a line. A short, consistent change request makes you state the window, the production impact and the way back before anyone touches a system.

**Sections:**

#### How to use the template

**Body:** Fill in the fields, review the generated document and export it to PDF or Word for the change advisory board. The blocks lead the requester through scope, benefit, steps, tests, impact and rollback. The risk and communication tables are for the CAB to complete or correct in the meeting. Keep a copy with the change record and close it with the actual outcome.

**Bullets:**

- Schedule windows around shifts, shipping deadlines and month end
- Name a rollback decision time, not just a rollback plan
- Include the MES, EDI and reporting interfaces in the impact
- Record the result after the change

#### Standard, normal and emergency changes

**Body:** A standard change is low risk and pre-approved, such as a routine patch that follows a documented procedure. A normal change needs review by the CAB. An emergency change addresses a serious incident or security issue and is approved quickly by a smaller group, with the paperwork finished afterwards. Defining these types prevents every request from going through the same slow path.

#### What a good rollback plan contains

**Body:** It states the trigger, the person who decides, the time by which the decision must be made, and the exact steps. It also states how long rollback takes, because a rollback that needs three hours is no use if the window ends in two. Confirm that a backup exists, that it was verified and that restore has been tested, especially for ERP databases and virtual machines.

#### Common mistakes

**Body:** Approving changes with no tested rollback, scheduling during a shipping deadline, and forgetting related systems such as label printers, barcode scanners, EDI and reporting tools are the usual problems. Another is mixing several unrelated changes into one window, which makes the cause of any failure hard to find. Keep changes small and well described.

**Cta:** Netray can help build a private assistant that drafts change requests and checks them against your system dependency map. Start with the template if a lighter process is enough.

**Faq:**

#### What should a change request include?

**Answer:** Title, requester, reason, systems affected, planned window, implementation steps, test results, production impact, risk, rollback plan and approvals. For plants, add shift impact and who to call if the change overruns.

#### How often should a CAB meet?

**Answer:** Weekly is common for medium sites, with an emergency CAB that can be convened quickly. Small teams may combine it with an existing operations meeting as long as decisions are recorded.

#### Do all changes need CAB approval?

**Answer:** No. Pre-approved standard changes that follow a documented, low risk procedure can skip it. Everything with meaningful production risk, new interfaces or security impact should be reviewed.

#### Is this template valid for regulated systems?

**Answer:** It is a starting point. Systems under quality regulation may need validation records and approvals from quality, which this form allows for but does not replace. Follow your own quality system.
