Required by: {{needed_by}}
Project: {{project}}
NDT contractor: {{contractor}}

#### Examination details

**Body:** Method: {{method}}
Weld stage: {{stage}}
Procedure and standard: {{standard}}
Acceptance criteria: {{acceptance}}

#### Joints to be examined

**Body:** {{joints}}

#### Readiness and access

**Body:** {{access}}

The requester confirms that visual examination has been completed and accepted before this request, and that welds are clean and free of paint, slag and spatter in the examination area.

#### Safety requirements

**Body:** {{safety}}

#### Joint schedule

**Table:**

**Columns:**

- Joint ID
- Weld length
- Thickness
- Extent %
- Notes

**Rows:**

#### Approvals

**Signatures:**

- Requested by
- Welding inspector
- NDT contractor acknowledgement

**Notes:** A generic call-out form. Include the NDT procedure number, personnel qualification requirements (for example ISO 9712 or SNT-TC-1A as written in your contract) and any client hold points. Radiography requires regulatory controls on site; follow local rules and your radiation safety officer.

## Content

**Intro:** A vague NDT request wastes a technician's visit. This template collects the information inspectors need before they arrive: method, procedure, acceptance criteria, joint list, access and safety.

**Sections:**

#### How to use the template

**Body:** Fill in the project data, choose the NDT method and the weld stage, and reference the NDT procedure and acceptance criteria from the contract. List joint IDs exactly as they appear in the weld log, and add the joint schedule table with lengths, thickness and extent. Describe access and surface condition honestly, because a technician cannot test a rough or covered weld. Export to PDF and send to the NDT contractor, and keep a copy with the job file.

**Bullets:**

- Request only after visual inspection is accepted
- Give at least one working day notice for UT and MT, more for RT
- State whether post weld heat treatment has been completed
- Mention any previous repairs on the joint

#### Choosing the method

**Body:** Surface methods like MT and PT find defects open to the surface, with MT limited to ferromagnetic materials. UT and RT examine the volume of the weld. UT is sensitive to planar flaws such as lack of fusion and cracks, while RT is stronger on volumetric flaws like porosity. Phased array and TOFD add sizing and recording. Delayed hydrogen cracking can appear a day or more after welding, so some specifications require a hold time before examination of susceptible steels. State the hold in the request if applicable.

#### Reading the example

**Body:** The sample request calls for UT after post weld heat treatment on four longitudinal seams of a pressure skid, citing a procedure and an acceptance basis. Access notes confirm that caps are ground flush for UT and the safety notes state no special permits are needed. Replace the sample text with your job. The signature block ensures the request is traceable to both the welding inspector and the contractor.

#### Common mistakes

**Body:** Failed inspection visits are usually preparation problems.

**Bullets:**

- Welds not cleaned or scaffolding not ready on arrival
- No acceptance criteria stated, so the report cannot say accept or reject
- Wrong joint IDs, causing the wrong weld to be tested
- Requesting NDT before PWHT when the specification requires it after
- No note about radiography exclusion zones for neighboring crews

**Cta:** If NDT call-outs and results are tracked in spreadsheets, we can show how they could be connected to work orders and quality records in your ERP.

**Faq:**

#### What information must an NDT request contain?

**Answer:** Project and joint IDs, method, procedure and standard, acceptance criteria, weld stage, access and surface condition, required date and safety requirements. Without these the technician cannot plan the visit or issue a report that can be accepted.

#### Should visual inspection come first?

**Answer:** Yes in most codes. Visual examination is usually required on all welds and should be accepted before volumetric or surface NDT is called. This avoids wasting time examining welds with obvious surface defects.

#### How much notice does NDT need?

**Answer:** It depends on the contractor and method. Surface methods can often be done the next day, while radiography needs site planning and safety controls, so give more notice. Agree lead times in the contract.

#### Can this be used for third party inspectors?

**Answer:** Yes. Add the third party's hold points in the notes and attach it to the call-out. The form records what was requested, while acceptance remains defined by the code and contract.
