## Content

**Intro:** An access request form is the paper trail behind every NetSuite login. Auditors ask for it, and administrators need it to answer the question of who approved a role and why. Fill in the fields, then export a PDF or Word copy.

**Sections:**

#### How to use the form

**Body:** Complete one form per person and per change. Pick the request type, list roles by their exact names as shown in Setup, Users/Roles, Manage Roles, and state the subsidiaries or locations the person needs. The business reason should describe the job task, not just the job title. After manager and process owner approval, the administrator provisions the access and ticks the checklist before the form is filed.

**Bullets:**

- One form per person per change
- Use exact role names from your account
- Record the end date for any temporary access
- File the signed copy with your access review evidence

#### Segregation of duties in NetSuite

**Body:** NetSuite roles are made of permissions with levels such as View, Create, Edit and Full. Conflicts usually come from combining permissions, for example creating vendors and approving vendor payments, or editing item costs and posting inventory adjustments. Write down the check you actually performed in the SoD box. If your company uses a tool or a saved search of role permissions to test combinations, name it. A blank SoD box is a common audit finding.

#### Manufacturing roles to watch

**Body:** In manufacturing accounts the sensitive permissions tend to be work order and assembly build entry, inventory adjustment, item cost editing, and BOM and routing maintenance. A planner who can both release work orders and change the BOM that drives them may be acceptable at a small plant and a concern at a larger one. Record the reasoning either way so the next reviewer does not have to rediscover it.

#### Common mistakes

**Body:** The most frequent problems are shared logins, copying the roles of a colleague without review, and forgetting removal when someone changes teams. Treat a change of role as a removal of the old access plus a grant of the new. Use the Remove access request type on leaving dates and review the log of open requests monthly.

**Cta:** If you want user and role reviews pulled automatically from NetSuite and compared with your approved forms, Netray can discuss a read-only reporting setup.

**Faq:**

#### Do I need a separate form for temporary access?

**Answer:** Use the same form and choose Temporary access with an end date. The end date is what lets the administrator schedule removal and gives the auditor evidence that the access did not stay open indefinitely.

#### Who should approve a NetSuite access request?

**Answer:** Typically the requesting manager and the owner of the business process, such as the controller for finance roles. The administrator provisions but should not be the only approver for their own request.

#### Is this form enough for SOX?

**Answer:** It is a useful piece of evidence, not a full control. SOX also expects periodic access reviews, termination handling and change logs. Check the wording with your auditors before relying on it.

#### Can I edit the checklist steps?

**Answer:** The checklist is fixed in the exported document, but you can edit the downloaded Word copy to add steps such as sandbox access or SuiteAnalytics Workbook permissions that apply in your account.
