**Notes:** This is a generic planning template based on common lean A3 practice. Adapt the headings to your company standard. Keep the OEE definition and ideal rate identical between baseline and results, or the comparison is not valid.

## Content

**Intro:** An A3 is a one-page problem-solving format from lean practice. This template adapts it to an OEE project: baseline, goal, root cause, countermeasures with owners, a before and after loss table, and the follow-up that makes the gain stick. Fill the fields, and export a clean document to print, share or attach to a funding request.

**Sections:**

#### How to use the template

**Body:** Start with data, not opinions. Fill in the baseline OEE from a quarter of data and the biggest loss category from your loss pareto. Write the background as a short statement of what hurts: late orders, overtime, rising cost. State the goal with a number and a date. Complete root cause analysis with the team at the line, using five whys or a fishbone. Then add countermeasures in the table with a single owner and a due date each. Print the page and keep it at the cell.

**Bullets:**

- One A3 per loss, not per machine
- Baseline and goal must use the same OEE definition
- Every countermeasure gets one owner and one date

#### Why this structure works

**Body:** The A3 forces a short story: what is the problem, what is the current condition, what is the goal, why does the problem exist, what will we do, who does what by when, and how will we know it worked. The two tables are the working part. The countermeasures table converts ideas into commitments. The loss table shows whether the OEE gain came from the loss you targeted, or from something unrelated like a change in product mix. Without it, teams cannot tell which action worked.

#### Worked example

**Body:** The sample shows a cell at 62 percent with setup as the biggest loss. The root cause is not the machine at all: tools are fetched during the stop, there is no standard sheet and trial pieces queue at a remote CMM. Countermeasures follow directly: kitting, a standard sheet and a pre-booked CMM slot. Target 72 percent in about three months. Price the benefit with the OEE payback calculator and break the setup into elements with the SMED worksheet before presenting.

#### Common mistakes

**Body:** Jumping to countermeasures before the root cause is agreed produces tool purchases that do not fix the process. Choosing a goal without a date makes the A3 a wish. Another frequent mistake is closing the project when OEE improves for a month: check the standard work is updated, the ERP routing and standard times reflect the new method, and the result holds for at least a quarter before you declare it done.

**Bullets:**

- Agree the root cause before choosing countermeasures
- Always set a date and a measure for the goal
- Update standard work and ERP master data at close

**Cta:** If the baseline data is hard to get because production, downtime and quality sit in different systems, we can help join them in a read-only view for the project team.

**Faq:**

#### What does A3 mean?

**Answer:** It refers to the A3 paper size, roughly 11 by 17 inches. Toyota popularized using one sheet to tell a complete problem-solving story. The format matters less than the discipline of being brief and visual.

#### Who should own the charter?

**Answer:** Someone close to the line, such as a supervisor or process engineer, with a sponsor who can remove obstacles. Ownership by a distant manager usually means the line team treats the A3 as paperwork.

#### How long should an OEE project last?

**Answer:** A single loss project often fits in 8 to 12 weeks. Longer than that, split it. Smaller, faster wins keep the team engaged and make the results easier to attribute to specific changes.

#### Can I edit the headings?

**Answer:** Yes. Export to Word and change anything that does not match your company's A3 or kaizen form. The structure is a starting point, not a mandated format.
