**Notes:** Replace names with privacy-protected entries where 29 CFR 1904.29 allows (for example privacy concern cases). Count days using calendar days, stop counting at 180 days per case, and do not count the day of injury. Use the recordability rules in 29 CFR 1904 to decide what goes on the log. This worksheet is a convenience layout, not the official OSHA form.

## Content

**Intro:** The OSHA 300 log is the running record of work-related injuries and illnesses that meet the recordkeeping criteria. This worksheet mirrors its main columns and totals so you can build the annual 300A summary from it.

**Sections:**

#### How to use the worksheet

**Body:** Add one row per recordable case within seven calendar days of learning about it. Enter the employee, job, date, where it happened, a short description including the body part, and the most serious outcome. Fill days away and days of restricted work or transfer. The sheet totals cases and days and computes lost and restricted days per row. At year end, transfer the counts by outcome and injury type to the official 300A summary, certify it by an executive, and post it from February 1 to April 30. Establishments with 20 to 249 employees in covered industries submit data electronically.

#### Recordability and counting rules

**Body:** A case is recordable if it is work-related and results in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, loss of consciousness, or a significant diagnosed injury or illness. Days away and restricted days are counted in calendar days, not scheduled workdays, and counting stops at 180 days per case. The day of injury is not counted. Cases from different types go into separate columns on the official form. Keep the log for five years following the end of the calendar year it covers.

**Bullets:**

- Record within seven calendar days
- Count calendar days, cap at 180 per case
- Keep the log five years after the year it covers
- Protect employee privacy on privacy concern cases

#### Common mistakes

**Body:** The classic errors are treating a case as first aid when it received prescription-strength medication, failing to update days away when an employee remains out, counting scheduled workdays instead of calendar days, and forgetting to log a case that started as first aid and later worsened. Another is not reconciling the 300 log with workers' compensation claims and the first report of injury. Reconcile them each quarter and correct entries as facts change. This feeds directly into your TRIR and DART rates.

**Cta:** If you want incident data from your ERP, HR and workers' compensation systems consolidated into this log automatically, Netray can scope the integration.

**Faq:**

#### Is this the official OSHA 300 form?

**Answer:** No. It follows the main columns of Form 300 to help you organize data and prepare the 300A summary. Use the official OSHA forms or the OSHA Injury Tracking Application where required to submit.

#### How long must I keep the log?

**Answer:** Five years following the end of the calendar year the records cover. Update the log during that period to reflect new information, such as additional days away.

#### Which employers must keep an OSHA 300 log?

**Answer:** Many employers with more than ten employees in industries not on the partially exempt list. Check 29 CFR 1904 appendices for your NAICS code and your state's plan.

#### What is the 180 day cap?

**Answer:** You may stop counting days away or restricted days at 180 for any one case. The log shows the cap rather than the actual number once it is reached.
