**Notes:** Score is probability times impact on a 1 to 5 scale, giving 1 to 25. A common convention treats 15 or more as high, 8 to 14 as medium and under 8 as low. Add rows for your own risks and review the register at every steering meeting.

## Content

**Intro:** A risk register only helps if it is short, scored and reviewed. This worksheet starts with realistic S/4HANA migration risks that you can edit, score and export for the steering committee.

**Sections:**

#### How to use it

**Body:** Replace or edit the example rows with the risks of your program. Give each a category, a probability and an impact on a 1 to 5 scale, an owner and a mitigation that is an action, not a hope. The score column multiplies probability by impact. Sort or review from the highest score down, and set a review date for each risk. Export to Excel to share with the PMO.

**Bullets:**

- One owner per risk, by name or role
- Mitigation must be an action with a date
- Re-score after every mitigation
- Close risks that no longer apply

#### The scoring method

**Body:** Probability times impact is the classic qualitative approach. Both scales run 1 to 5, so scores range from 1 to 25. Treat the numbers as ordering aids rather than precise values. The summary line shows the average score and the totals show the number of risks and the highest score. A rising average between reviews is an early warning even when no single risk crosses your threshold.

#### Typical S/4HANA risk areas

**Body:** Frequently recurring risks include data quality and business partner conversion, custom code remediation, integration testing capacity, cutover duration, key user availability, interface partners, performance on production-sized data, and vendor staffing changes. Tailor the list by adding risks specific to your regulatory environment and plant structure. Link each mitigation to a workplan task so it is tracked, not just written.

**Cta:** Netray can build a private assistant that reads your status reports and logs and highlights risks that are drifting.

**Faq:**

#### How many risks should a register have?

**Answer:** Keep an active list of about 15 to 40 meaningful risks. A register with hundreds of items is rarely reviewed, and one with five rarely covers the program.

#### What is the difference between a risk and an issue?

**Answer:** A risk may happen in the future. An issue has already happened. Move a risk into your issue log when it occurs and track the response there.

#### Who should own a risk?

**Answer:** The person best placed to act on it, not necessarily the program manager. Data risks go to the data lead, cutover risks to the cutover manager, and so on.

#### How often should I review?

**Answer:** At least every two weeks, and weekly during test and cutover phases. Review the top ten at every steering committee meeting.
