## Content

**Intro:** A clear spill record shows an auditor or regulator that you responded promptly, found the cause and closed the actions. This template gives your team a structure to complete while the details are fresh.

**Sections:**

#### How to use the template

**Body:** Complete the top fields as soon as the spill is under control: where, when, what material and how much. Describe what happened in plain language and list each response step in order. Use the notification table to record who was told, including internal managers, regulators and any customer with a contractual duty. Finally, assign root causes and corrective actions with owners and dates, then export the document to PDF or Word for your records.

**Bullets:**

- Attach the safety data sheet for the material
- Attach photos and a sketch of the area
- Record whether the spill reached a drain or soil
- Keep waste manifests with the report

#### Why the details matter

**Body:** Regulators often ask whether a spill exceeded a reportable quantity and whether it reached the environment, so those two facts must be clear. ISO 14001 also expects emergency preparedness and response to be tested and reviewed after incidents. A complete report supports the review and shows that the process works.

#### Finding the root cause

**Body:** Do not stop at the immediate cause such as a failed hose. Ask why the hose failed, why no one noticed and why the area did not contain more. Typical root causes include missing inspection, wrong fitting, poor training, no secondary containment or a rushed changeover. Write each corrective action so that it can be verified.

#### Common mistakes

**Body:** Writing the report days later from memory, estimating quantity without a method, and closing actions without evidence. Take an estimate by container level or area and depth, and state the method.

**Cta:** If incidents and corrective actions are tracked in your ERP or QMS, we can help link environmental reports to the same workflow.

**Faq:**

#### Do I need to report every spill to the authority?

**Answer:** No. Reporting depends on the substance, quantity and where it ended up. Small contained spills are often internal only, while releases over a reportable quantity or to water or soil usually must be reported quickly. Confirm the thresholds for your site with your permit and regulator.

#### How do I estimate spill quantity?

**Answer:** Use the change in container level, the volume of the leaking vessel, the area covered times average depth, or the amount of absorbent used. Record the method and note that the figure is an estimate.

#### How long should spill reports be kept?

**Answer:** Follow your documented information procedure and any legal retention period. Many sites keep environmental incident records for at least three to five years, and longer when a permit or contract requires it.

#### Should near misses be reported too?

**Answer:** Yes. A hose that nearly failed or a drum that tipped without leaking shows the same weakness. Near miss reports let you fix the cause before a real release, and they are good evidence of a working EMS.
