#### Approvals

**Signatures:**

**Notes:** Record the severity using your own site alarm limits or the zone limits you adopted from ISO 20816 (which replaced ISO 10816) for your machine class. Attach spectra and waveforms as images to the exported file. This template is a starting point and does not replace your analyst certification procedures.

## Content

**Intro:** A vibration report is only useful if the maintenance planner can act on it without calling the analyst. This template forces the useful fields: what was measured, what the spectrum showed, what the fault probably is, how urgent it is and what to do next.

**Sections:**

#### How to use the template

**Body:** Fill in the header fields, type your overall readings, then write the findings in plain engineering language. Keep the diagnosis to one or two sentences and state your confidence. Choose a severity from your site scale, add the recommended action with a re-survey date, and export to PDF or Word. Paste the exported text into the notification or work order in your CMMS so the report and the job stay linked.

**Bullets:**

- Always record running speed and load, a spectrum without them is hard to compare later
- Use the same point names and directions every survey
- Tie each report to an asset ID that matches your CMMS equipment record

#### What a good diagnosis looks like

**Body:** Tie each conclusion to evidence. Misalignment shows elevated 1X and 2X with a strong axial component. Unbalance shows a dominant radial 1X with little axial. Looseness shows harmonics and sometimes sub-harmonics. Rolling bearing defects appear at non-synchronous frequencies and in acceleration or envelope spectra. A diagnosis that names the frequency and the direction is easy to challenge and easy to confirm after the repair, which is how analysts build credibility.

#### Reading severity

**Body:** Overall velocity severity depends on machine class, mounting and your own history. Published guidance in ISO 20816 gives zone boundaries by machine group, but a trend of doubling over a few surveys matters more than any single number. Record the trend column in the table and escalate on rate of change as well as absolute level. Many teams set alert at 1.5 times the baseline and alarm at 2.5 times the baseline until they have enough data.

#### Common mistakes

**Body:** The most frequent problems are reports without a stated action, no follow-up date, severity assigned from one reading, and analysis done on a machine that was running off normal load. Another is mixing units, for example comparing mm/s RMS with in/s peak. Fix the unit in your procedure and print it in every table header.

**Cta:** If your vibration findings live in PDFs and never reach the ERP maintenance module, Netray can help connect survey results to equipment records and work orders.

**Faq:**

#### Is this template tied to a specific analyzer brand?

**Answer:** No. It is brand neutral. Type the numbers from any portable analyzer or online system, and attach screenshots of spectra from your software to the exported document if you want the plots included.

#### Which standard should I cite for severity?

**Answer:** ISO 20816 is the current family for evaluating machine vibration, replacing ISO 10816. Many sites also use their own limits derived from baselines. Whatever you use, state it in the report so readers know what the label means.

#### How often should the report be issued?

**Answer:** Issue one after every route survey where an asset is in Watch or worse, and a summary for healthy assets. Frequency of the surveys themselves follows your P-F interval and asset criticality.

#### Can I export it to Word?

**Answer:** Yes. Every template here exports to PDF and Word automatically, so you can edit wording further or paste it into a CMMS notification.
