OJT Training Sign-Off Record Template
Auditors and customers ask the same question: how do you know this person is competent to do this task? A signed OJT record answers it with evidence tied to a specific procedure, trainer and date.

Adapt the step table to the actual work instruction. Keep signed originals with the employee training file for the retention period your quality system and local law require.
Link training records to ERP authorizations
An optional review of how to restrict work center reporting to qualified operators.
Optional. The tool and every export work without this. No spam.
How to use the record
Create one record per task or procedure. Replace the example steps with the actual steps from the work instruction, mark each as demonstrated, practiced and competent as training progresses, and sign when the trainer has seen the trainee perform the task correctly without help. Export to PDF for the training file, or keep the Word version for editing. File a copy where the supervisor can see which stations the person may run.
- One record per task, not one per person
- Reference the revision of the work instruction used
- Record limits, such as one machine only
- Re-sign when the instruction changes materially
What standards expect
ISO 9001 clause 7.2 requires you to determine necessary competence and keep documented evidence of it. IATF 16949 and AS9100 add expectations for training on quality requirements and, in aerospace, awareness of product safety and ethical behavior. None of them prescribe a form. A record that names the person, the task, the trainer, the method, the date and the outcome meets the intent and is easy to audit.
Qualifying the trainer
A competency record is only as credible as the person who signed it. Keep a short list of qualified trainers per task, usually experienced operators or leads who hold the sign-off themselves. Avoid having trainees sign off their own peers on safety-critical or special-process tasks unless your procedure explicitly allows it.
Common mistakes
Frequent problems are signing the form at the end of a shift without observing the task, using one form for many tasks, and failing to link the record to the ERP or MES authorization. If your system restricts who can report on a work center or run a routing step, update that permission only after the signature exists.
Frequently Asked Questions
Is this enough for an ISO 9001 audit?
It gives you the documented evidence of competence the standard asks for, provided the steps match your work instruction and the records are retained. The auditor will also look at how you decide what competence is needed.
How long should I keep the records?
Keep them at least for the employment period plus any retention your customer contracts, regulators or quality system require. Aerospace and defense customers often specify longer periods, so check your flow-down terms.
Can the trainee sign for themselves?
The trainee should sign to acknowledge the training, but the competency decision belongs to a qualified trainer or supervisor. Two separate signatures keep the roles clear.
What if the trainee is not yet competent?
Choose the retrain result, record what was missing, and set a new date. Do not delete the earlier attempt. A visible history of retraining is stronger evidence than a spotless record.
If you want station authorizations in your ERP to follow completed training records automatically, we can help design that control.
