Supplier Onboarding Form Template

A new supplier form is the gate between a name on a quote and a vendor record in your ERP. Done well, it captures the right data once, prevents duplicate vendors and catches bank fraud before the first payment goes out.

Adapt tax and banking fields to your country (GSTIN and PAN for India, VAT ID in Europe, W-9 in the United States). Always verify bank changes independently, since fake supplier bank detail requests are a common fraud method. This template is not legal advice.

Validate vendor data before it hits the ERP

Optional: we can outline duplicate and bank-detail checks that run on new vendor records in your ERP.

Optional. The tool and every export work without this. No spam.

How to use the template

Fill the fields from the supplier's information, preview, and export as PDF or Word to send for completion or signature. The form follows the order of work: company identity, commercial terms, quality and compliance, documents, then internal checks by the buyer. Edit the document checklist for your country and category, for example add an export license declaration for controlled items or a food safety certificate for packaging that touches food.

What to collect and why

Each data point should earn its place. Legal name and tax number support duplicate checks and statutory reporting. Bank details drive payment, so they need verification outside the form. Quality certificates determine approval status, and insurance proves the supplier can cover liabilities. Collect only what you use, and protect the data, since onboarding forms hold sensitive personal and financial information.

  • Legal entity details and ownership changes
  • Tax registration, W-9 or W-8, GSTIN, VAT ID as applicable
  • Bank account verified by call-back to a known number
  • Quality certificates with expiry dates
  • Insurance, code of conduct and sanctions screening

Common mistakes

Skipping the duplicate check creates two vendors for the same supplier and split payment history. Accepting bank changes by email is the biggest fraud risk, so use a call-back control and segregate the person who creates the vendor from the person who approves payments. Another mistake is collecting forms but never refreshing them: set a periodic review so expired certificates and changed details are caught.

ERP vendor master notes

Map form fields to your vendor master before you send it out: business partner and company code data in SAP, supplier site and bank account in Oracle, vendor record and remit-to in SyteLine, Epicor and Dynamics. Making the form match the ERP fields saves rekeying. Require approval workflow for creation and any change to bank details, and keep the signed form as the audit trail.

Frequently Asked Questions

What information is required to set up a new supplier?

At minimum the legal name, address, contact, tax identifier, payment terms, currency and verified bank details. Add quality certificates, insurance and compliance declarations as your risk and industry require, and attach supporting documents.

How do I prevent supplier bank fraud?

Verify new or changed bank details with a call-back to a number already on file, not one from the email. Separate vendor creation from payment approval, and review changes in a report. Never accept changes only by email.

Should the supplier fill in the form or my buyer?

The supplier should complete company, tax and bank sections and sign, while the buyer completes internal checks. Supplier-provided data is more accurate and creates evidence that the supplier confirmed the details.

How often should supplier details be refreshed?

Review annually for active suppliers and whenever certificates expire. Trigger an immediate review after a change of ownership, address, bank account or a serious quality or compliance event.

If vendor setup and change requests are slow or error-prone in your ERP, Netray can help design checks and agents that validate new vendor data before it is saved.
