A starting point, not legal advice. Check your supply agreement for the warranty period, exclusions, and who bears freight on returned parts before you apply this form.

Automate warranty intake

Optional review of linking warranty claims to serial numbers and service orders.

Optional. The tool and every export work without this. No spam.

How to fill in the form

Start with identifiers: claim number, machine model and serial number. The serial number links the claim to the build record, commissioning date and any configuration changes. Enter run hours, because many machinery warranties limit coverage by hours as well as months. Describe the failure in observable terms: what the fault code said, what the machine was doing, and what was tried. Add operating conditions so the OEM can rule out misuse. Fill the evidence table and attach photos and logs before sending.

  • Serial number, not just model, on every claim
  • Quote the exact fault or alarm text
  • Attach controller or drive logs where available
  • State what the operator already tried

What the OEM checks

The warranty administrator confirms three things: the machine is inside the warranty period and hour limit, the failure is a defect and not wear, misuse or an excluded cause, and the part is a covered component. Consumables, wear parts and damage from incorrect power or environment are typically excluded. A clear operating conditions section speeds this up. For a disputed claim, request the failed part back for analysis and record the result in the disposition block.

Using the claim data

Over time, claims show which parts fail early, which models drive most cost and which customers misuse equipment. Export each form to Excel and keep a register by part number and failure mode. A pattern of the same drive failing before 3,000 hours is an engineering issue, not a service issue, and should feed a corrective action. Pair the register with a warranty reserve calculation so finance books a realistic provision.

Common mistakes

Frequent errors include claims without serial numbers, vague descriptions such as machine not working, and no photos. On the OEM side, delays happen when the form is not tied to a service order or when the decision is communicated by phone only. Record the decision on the form, store it with the service order, and send a copy to the customer so the history is complete.

Frequently Asked Questions

What information is mandatory on a warranty claim?

Serial number, commissioning date, run hours, failed part, a clear failure description and evidence such as photos or fault logs. Without these the OEM usually cannot confirm coverage and the claim stalls.

Does the customer need to return the failed part?

Often yes, so the OEM can analyze the root cause and claim against its own supplier. Say in the disposition whether return is required and who pays freight, following your supply agreement.

How long should a claim take?

Set targets in your agreement, for example acknowledgment within one business day and a decision within a few days once evidence is complete. Missing information is the main cause of delay.

Can this form be used for spare parts claims?

Yes. Use the failed part field for the spare, enter the machine serial number it was installed on and the install date in the operating conditions, then follow the same decision steps.
