Management Report Request Form Template

Most report projects fail at the request, not the build. This form captures what a developer or ERP administrator needs to build the report once: the question, the audience, the fields, the filters and how to know it is right.

How to use the form

Start with the business question, written in plain language. Name who will read the report and how often, then list the fields and the filters users will need. Fill in the definitions table for any term that could be read two ways, such as late, open or shipped. Finish with an acceptance test that ties the new report to a number you already trust. Export to PDF or Word and attach it to the work ticket.

Why definitions and acceptance tests matter

Rework usually comes from two sources: ambiguous terms and no way to verify. If planning measures late against the promise date and sales against the request date, both will call the report wrong. An acceptance test, such as a reconciling total to an existing report at the same cutoff date, ends the argument before go-live.

  • State the cutoff date logic (as of date vs live)
  • Name the included sites, companies and order types
  • Say how currency and units of measure are handled
  • List who is allowed to see cost and pricing columns

Common mistakes

Do not ask for 'everything' and filter later; wide reports are slow and confusing. Do not skip the owner: every report needs a named person who answers questions about it and retires it. Check whether an existing standard report or saved view already meets the need, since a new custom report adds maintenance and upgrade testing. Consider security roles early, so the report respects ERP permissions.

Frequently Asked Questions

Who should approve a report request?

The requester's manager approves the need, and the data owner or ERP administrator approves access. For reports that show cost, pricing or controlled data, add a security or compliance sign-off.

How detailed should the field list be?

List business names, not table columns. The developer maps them to ERP fields. Add a sample row if you can, because an example removes most misunderstandings in seconds.

Should I request a report or a dashboard?

Use a report for detail someone will export or audit. Use a dashboard for a handful of numbers watched often. If unsure, start with the report, then promote the chart your users keep rebuilding.

What is a good acceptance test?

A reconciliation to an existing trusted figure at the same cutoff date, plus a spot check of three records traced to the source screen. Write it before development begins.
