Safety Training Matrix and Expiry Tracker Worksheet

Training that lapses is one of the easiest findings in an audit and one of the easiest to prevent. This worksheet keeps every employee and requirement in one table and flags what is overdue or coming due.

Default intervals are common program values, not legal advice: forklift operator evaluation at least every 3 years (OSHA 1910.178), annual respirator fit test (1910.134), annual audiogram in a hearing conservation program (1910.95). Confirm the interval for your site, state and customer requirements.

Optional review of how training status can follow employees and work centers.

Optional. The tool and every export work without this. No spam.

How to use it

Add one row per employee and requirement. Enter the required refresher interval in months and the months since the person last completed it. The sheet computes months remaining, marks overdue items and flags those due within two months. The summary shows what share of items are current. Sort by months remaining in Excel after export to build a schedule for the next training block. Update the months since column monthly, or replace it with completion dates in your own file if you prefer.

  • One row per person per requirement
  • Use the strictest interval that applies
  • Review the due soon list every month

Where intervals come from

Some intervals are set by regulation and others by company policy or customer requirement. OSHA requires an evaluation of powered industrial truck operators at least every three years, annual respirator fit testing for users, and annual audiograms inside a hearing conservation program. First aid and CPR certificates are usually valid for two years from the issuing body. Confined space and lockout retraining is triggered by changes in procedures or evidence of gaps, so many plants set an annual review cycle as policy.

Reading the summary

The share of items current is a simple compliance score, but treat each overdue item on its own. A lapsed fit test on a person who uses a respirator daily is a more urgent problem than a late refresher on a low-risk topic. Use due soon to book classes early and avoid pulling people from production at short notice. If the same course is often late, the cause is usually scheduling capacity, not individuals.

Common mistakes

Tracking only the date of the original course, not the retraining trigger, is common. Others are forgetting contractors and temporary staff, mixing calendar months with training windows and keeping the matrix in a file only one person can edit. Keep a master version, version it and link it to job roles so new starters appear with their required courses automatically.

Frequently Asked Questions

What is a safety training matrix?

It is a table that maps employees or job roles to the training they need, when they last completed it and when it expires. It is used for audits, scheduling and for deciding who may be assigned to a hazardous task.

How often must forklift operators be re-evaluated?

OSHA 1910.178(l)(4) requires an evaluation of each powered industrial truck operator's performance at least once every three years. Refresher training is also needed after an accident, near miss, unsafe operation or change in conditions.

Can I add my own courses?

Yes. Type any text in the course cell if it is not in the list, or edit the rows freely. Exports to Excel keep your changes, so you can build the full matrix there.

How do I handle new starters?

Add a row for every required course with months since set equal to the interval or higher until they complete it. That shows the item as overdue and makes the gap visible from day one.

If training records live in HR, a learning system and spreadsheets, we can help connect them to roles and work centers in your ERP so assignments and expiry alerts follow the job.
