Wellhead and Christmas Tree Factory Acceptance Test (FAT) Template

A factory acceptance test record is the proof package customers expect before wellhead or tree equipment leaves the shop. This template structures the pre-test checks, pressure tests, function tests and sign-off so the data book is complete the first time.

Test pressures, hold times and acceptance rules come from your governing specification and the customer's datasheet. Enter the numbers from those documents rather than relying on defaults here. This is a record template, not an engineering approval.

How to use this template

Fill in the header with the PO, serial number and governing specification, then enter the rated working pressure, medium and hold time. Print the pressure and function tables for the test crew, or complete the exported Word document directly. Attach gauge calibration certificates and pressure charts as appendices, and have the witness sign before the equipment is released to shipping.

What a typical FAT covers

Equipment built to API Spec 6A is normally verified by hydrostatic body tests, seat and sealing tests, and where specified a gas test, with the product specification level driving the extent of testing. A dimensional and traceability check precedes pressure testing, and valves are cycled through open and close to prove function. The specification, not this template, defines the pressures and durations.

  • Verify traceability and dimensional records first
  • Use calibrated gauges and a pressure chart recorder
  • Hold at pressure for the time stated by the specification
  • Record any leakage, even if the retest passes

Common mistakes

Frequent problems are missing gauge certificates, starting the hold timer before pressure stabilises, and accepting a retest without recording the first failure. Another is testing with the wrong end connection ring gasket, which later gets replaced and invalidates the record. Keep the traveler, chart recorder trace and this form together so a customer audit can match each test to a time and operator.

Frequently Asked Questions

Does this template satisfy API 6A?

It supports the record keeping, but compliance depends on your quality program, the product specification level and the actual test procedure. Use it alongside your approved procedures and the customer's requirements.

Who should witness the FAT?

That is set by the contract. Some customers send their own inspector, others delegate to a third party, and some accept a manufacturer record. The witness field lets you document whichever applies.

Do I need a gas test?

Only if the specification or customer requires it. Gas testing carries more stored energy and needs specific safety controls, so it is typically done in a protected test cell with a documented procedure.

Where do pressure charts go?

Attach them to the record and cross-reference the serial number and time on the chart. Many customers require the original chart or a legible copy as part of the final data book.

If test results, serial numbers and customer data books are assembled by hand from several systems, we can discuss assembling them automatically from your ERP and quality records.
