8D Problem Solving Report Template

The 8D method gives a team a fixed path from a customer complaint to a verified fix: contain, find the cause, correct, prevent, close. This template lays out each discipline as a block you fill in, so the report reads the way automotive and industrial customers expect.

8D Problem Solving Report

Company: Acme Precision Ltd Report number: 8D-2026-031 Date opened: 2026-10-20 Customer / requester: Northline Drivetrain Part: PN-48213 Housing, main bearing

D1 Establish the team

Quality engineer (leader), Machining supervisor, Process engineer, Supplier quality, Customer liaison

D2 Describe the problem

Bore diameter 25.04 to 25.06 mm against 25.00 +0.02 on 12 of 80 pieces in lot L-5531, found at customer incoming inspection on 2026-10-14. Not seen in prior 6 lots.

D3 Interim containment

100 percent sort of stock at plant (420 pcs) and in transit (300 pcs). Certified sort at customer. Tool offset check added each hour until the permanent fix is verified.

D4 Root cause analysis

Occurrence: boring bar insert wear exceeded offset update interval. Escape: in process check sampled 1 in 50, wear drift cleared the check between samples.

D5 and D6 Permanent corrective action and verification

Chosen actions: Insert life limit set to 200 pcs with automatic tool change. Sampling changed to 1 in 10 with run chart on bore diameter.

Implementation and validation: Implemented 2026-10-28. Cpk of 1.67 over 125 consecutive parts. Work instruction WI-722 Rev D released.

D7 Prevent recurrence

Insert life limits reviewed across all boring operations. PFMEA and control plan updated. Lessons learned shared with other lines.

D8 Close and recognise the team

Closed after 3 clean lots and customer confirmation. Team acknowledged at monthly quality review.

D0 (symptom and emergency response) is often handled in the first containment step. Follow your customer's own 8D format if they mandate one, for example a customer portal form.

Link 8D reports to ERP quality data

Optional: we can review how your ERP stores rejects, lots and supplier data and outline how an 8D could be assembled from it.

Optional. The tool and every export work without this. No spam.

How to fill in an 8D report

Start with D1, naming a small cross functional team with a leader. D2 states the problem in measurable terms: what, where, when, how many, compared with what is normal. D3 protects the customer right away, with sorting, replacement or a tighter check, and records how long the containment runs. D4 separates the occurrence cause from the escape cause, since both need fixing. D5 and D6 choose and prove the permanent action with data. D7 spreads the learning to similar parts and documents. D8 closes the report and credits the team.

  • Quantify the problem in D2.
  • Show containment dates and quantities.
  • Find both why it happened and why it escaped.
  • Verify the fix with data before closing.

Choosing a root cause method

The 8D format does not prescribe a tool. For a simple direct cause, 5 whys is enough. For many possible causes, use a fishbone diagram to list candidates and test each with data. For intermittent or complex problems, compare good and bad parts, and use is and is not analysis. Whatever you use, test the cause: if you remove it, does the defect go away, and if you reintroduce it, does it return? A cause that is only plausible is not a verified root cause.

Common mistakes

Teams often jump from D2 to D5 and name operator error as the cause, which leads to retraining that rarely holds. Another mistake is leaving containment in place for months with no exit criteria. A third is closing at D6 without D7, so the same weakness remains in sister parts and processes. Customers also reject reports that give no dates, no owners or no evidence of effectiveness. Attach measurement data, updated control plan pages and the date containment ended.

Frequently Asked Questions

What does D0 mean in an 8D report?

D0 is the preparation step used in some versions of 8D: recognise the symptom, plan the response and decide whether an emergency action is needed. Many forms fold it into D2 and D3 and start numbering at D1.

How fast must an 8D be completed?

Customers set the clock. A common pattern is containment within 24 hours, a root cause in about 10 days and full closure in 30 to 90 days. Check your customer specific requirements and quality agreement.

What is the difference between 8D and CAPA?

8D is a team based problem solving format popular in automotive and industrial supply chains. CAPA is a broader quality system process in regulated industries. They overlap, and an 8D can serve as the record inside a CAPA.

Can I use this template for supplier problems?

Yes. Fill it as a record of your own response to a customer, or ask the supplier to complete it. For a formal request to a supplier, the supplier corrective action request template is a better fit.

If your 8Ds start from ERP quality notifications, we can help link the report to the lot, supplier and cost data automatically.
