PPAP Part Submission Warrant (PSW) Template

The part submission warrant is the signed cover sheet of every PPAP package. It identifies the part, states why you are submitting, declares the level and rate, and records a formal statement that the samples are representative. This template builds a clean summary you can export and attach.

Declaration

I affirm that the samples represented by this warrant are representative of our parts, and were taken from a significant production run. I affirm that all measurements and test results are included, and that the parts meet all customer design record and specification requirements: Yes.

I further affirm that these samples were produced at the production rate declared above, and that any deviation is explained below.

Explanation or comments

Dimensional results measured on 30 pieces from the run at rate. Capability studies attached for characteristics 14 and 22 (special characteristics).

This is a summary of the information a part submission warrant carries, not a copy of the AIAG form. Many customers require their own portal form or the AIAG PSW layout; use this output as a preparation sheet and transfer values accordingly. A starting point, not legal advice.

Pre-fill PPAP documents from ERP data

Optional: a short review of how part master, revision and routing data could populate PPAP paperwork.

Optional. The tool and every export work without this. No spam.

How to use it

Fill the fields from your engineering release and the purchase order. The part number and drawing revision must match the customer's current design record exactly, including the engineering change level. Choose the reason for submission, then the level that the customer specified. Enter the production rate from your run at rate, not the theoretical machine rate. Export to PDF or Word, review with your quality manager, and keep a signed copy in the PPAP file.

What the warrant declares

The signature is a statement of fact. The supplier affirms that parts came from a significant production run, using production tooling, gauges, process and rate, and that results meet all drawing and specification requirements. If any result is out of tolerance, the declaration must be answered No and the deviation explained in the comments, usually with a deviation request. Signing a warrant with undisclosed nonconformances is one of the most serious PPAP failures.

  • Samples from production tooling and the production process
  • Measured at the declared rate
  • All requirements met, or deviations explained
  • Signed by an authorized representative

Worked example

A stamping supplier submits a brake bracket at Level 3 for a new program. The reason is an initial submission, the run at rate covered two shifts at 480 pieces per hour, and 30 pieces were measured. Two special characteristics have capability studies attached. The comment field notes where the studies are in the package. The warrant is signed by the quality manager and uploaded to the customer portal with the other Level 3 elements.

Common mistakes

Wrong drawing revision, a blank engineering change field when a change exists, a rate that was never demonstrated, and ticking Yes on results while open nonconformances exist. Also watch the weight field, which is reported in kilograms to three decimals on many customer forms, and make sure the supplier code matches the one the customer uses in its own system.

Frequently Asked Questions

Who signs the part submission warrant?

An authorized supplier representative signs, typically the quality manager or plant manager. The signature carries legal weight because it declares that the samples are representative and the results are complete. Some customers also add their own approval block for disposition once the package is reviewed.

Is this the official AIAG PSW form?

No. It is a preparation summary that captures the same kind of information. Many customers require the AIAG layout or their own portal entry, so use this output to collect and review the data, then transfer it to the form the customer demands.

What reasons for submission exist?

Typical reasons are initial submission, engineering change, tooling transfer or replacement, correction of a discrepancy, and change of subcontractor, material source or process. Pick the one that matches why the customer asked for a new PPAP, and explain details in the comments.

What if a result does not meet requirements?

Answer No on the results statement and explain the deviation. Do not ship until the customer approves a deviation or you correct the process. Hiding a known nonconformance under a Yes declaration puts the supplier relationship and any later claims at risk.
