Corrective Action Report Format (CAR) with Sample and Process
A corrective action report (CAR) is the formal record that something went wrong, why, what was done and how it was proven. This format gives you the standard sections and a sample engineering CAR to edit and export.

Approvals
A CAR is the formal request for corrective action, usually raised from an audit finding, repeated nonconformance or complaint. Adjust severity levels and response times to match your quality procedure.

How to use this CAR format
Issue the CAR to the department that owns the process, not to the person who found the problem. Fill the header, describe the issue with evidence, and set a response date that matches severity. The responsible manager then completes the investigation, root cause and actions. Quality reviews the response, tracks the actions and closes the CAR only after the closure criteria are met. Edit the sample text, add your own approvals, and export to PDF, Word or Excel for the file.
The CAR process from issue to closure
A workable process has six steps: raise and log the CAR; assess severity; the owner contains and investigates; the owner proposes actions and dates; quality accepts or rejects the response; and effectiveness is verified before closure. Many companies set response times, for example a few working days for containment and a few weeks for the full response, and escalate overdue CARs in management review. The same flow supports ISO 9001 clause 10.2 and corresponding AS9100 and IATF 16949 expectations.
- Log every CAR with a unique number and source.
- Reject responses that stop at retraining without a systemic cause.
- Review open CARs weekly, not at the audit.
- Trend CARs by process, part and supplier.
Engineering CARs and common mistakes
Engineering CARs often come from release, change control or design review issues, so the root cause tends to be a missing gate or an unclear responsibility rather than a person. The sample shows this with a release checklist fix. Common mistakes are vague closure criteria, no link to the affected documents, and treating a CAR as complete when the form is complete rather than when the process is proven stable.
Frequently Asked Questions
What does CAR stand for in quality?
In most manufacturing quality systems CAR means corrective action report or corrective action request. It is the formal document used to record a problem, its cause, the actions and the verification of effectiveness.
What should a corrective action report contain?
A unique number, issue description with evidence, containment, root cause, corrective and preventive actions with owners and dates, and verification of effectiveness with sign-off. Severity and source are useful for trending.
How is a CAR different from an NCR?
An NCR records a nonconforming product or event and its disposition. A CAR addresses the cause of the problem to stop it recurring. One CAR can cover several NCRs with the same cause.
Can I download the CAR as a PDF?
Yes, use the export options on the tool for PDF, Word or Excel. The sample text is only an example and can be replaced.
Netray can add a private AI layer to your ERP and QMS data on your own servers so repeat CAR causes are visible across parts, processes and suppliers, with people approving all entries.
