Corrective Action Form
Company: Acme Precision Ltd Form no: CA-2026-041 Linked record: NCR-2026-0188 Date raised: 2026-10-12 Source: Customer complaint
1. Problem description
Part: PN-48213 Valve body, rev C Quantity affected: 38 of 400 pieces, lot 4471
Customer found 38 pieces with bore diameter 0.04 mm above upper tolerance at incoming inspection.
2. Containment
Quarantined lot 4471 and stock on hand. 100% sort completed on WIP. Customer notified, replacement lot shipped after sort.
3. Root cause
Boring tool wear was not detected because the in-process check was every shift, not by tool life. Why 5: no tool-life limit defined in the control plan.
4. Corrective action
Set tool-life limit of 400 pieces in the CNC program and add a bore check every 25 parts.
5. Action plan
| Action | Owner | Due date | Status |
|---|---|---|---|
| Set tool-life limit of 400 pieces in the CNC program and add a bore check every 25 parts. | A. Rao, Manufacturing Engineer | 2026-10-28 | Open |
| Update control plan and work instruction | |||
| Check similar parts and machines (preventive) |
6. Verification of effectiveness
Review SPC data and NCRs for this characteristic over the next 30 days of production. Zero out-of-tolerance results required.

How to fill in the corrective action form
Start with the linked record number so the form can be traced from NCR to closure. State the problem with a part, quantity, where it was found and what was wrong, using measurable terms. Containment describes what you did to protect the customer today. Root cause should go beyond the first answer; use 5 Whys, a fishbone or a fault tree and write the verified cause. Corrective actions fix that cause, and the action plan table assigns each one an owner and due date. The last section is the one auditors read first: how and when you will prove it worked.
- Use numbers: 38 of 400 pieces, 0.04 mm over tolerance.
- Separate containment (today) from corrective action (cause).
- Name a person and a date on every action.
- Define the effectiveness measure now, not at closure.
Example and procedure
The pre-filled example follows a typical machining complaint: oversized bores traced to tool wear with no tool-life limit. The sequence is the standard corrective action procedure used under ISO 9001 clause 10.2, IATF 16949 and AS9100: contain, find cause, act, verify, and update documents such as the control plan, FMEA and work instruction. A short procedure should also say who can raise a form, who approves closure, and the target days for each step so overdue items are escalated.
Common mistakes
Root cause written as 'operator error' with retraining as the only action, a verification line that says 'monitor', and no link to the original NCR are the three findings seen most often. Another is closing the form when the action is done instead of when it is proven effective. Keep a separate list of open forms so due dates are reviewed in the weekly quality meeting.
Frequently Asked Questions
What is a corrective action form?
It is the controlled record of a problem, its root cause, the actions taken to prevent recurrence, who owns them and how effectiveness was verified. It supports ISO 9001, IATF 16949, AS9100 and similar quality systems.
Can I get this as a PDF or Excel file?
Yes. Fill the fields on the page and export to PDF, Word or Excel. The example text is only a guide and can be overwritten completely.
What is the difference between correction and corrective action?
A correction fixes the nonconforming product, for example sorting or reworking it. Corrective action removes the cause so the problem does not return. Both belong on the form but they are different rows.
Who should approve closure?
Normally the quality manager or a person independent of the action owner, after reviewing the effectiveness evidence. Customer-specific requirements may add a customer sign-off for complaints.
If you want forms like this raised and routed automatically from NCR and receiving data in your ERP, Netray can build it as a private AI layer on your own servers with human approval on every record.
