DFMEA Worksheet Template

A design FMEA asks how a product could fail to perform its functions and what in the design could cause that. This template follows the function, failure mode, effect, cause chain and scores each line, so a review team can rank design risks before tooling is released.

Design FMEA focuses on how the design could fail to meet requirements, so causes are design or specification choices, not workmanship. Computed scores update when you edit S, O or D. Replace the examples with your own functions.

Optional: discuss keeping DFMEA lines linked to part revisions and change orders.

Optional. The tool and every export work without this. No spam.

How to use the template

Start from the block diagram or bill of materials and list the functions each item must perform, using a verb and a measurable requirement such as carries 5 A continuous. For each function add the ways it could fail, the effect at system and vehicle or customer level, and the design causes. Record prevention controls, which lower occurrence, separately from detection controls, which find the failure in test before release. Then rate S, O and D and fill the status column as actions progress.

  • Write functions with a requirement, not just a part name
  • Causes are design choices: material, geometry, tolerance, software logic
  • Prevention controls reduce occurrence, tests improve detection
  • Keep one owner and date per open action

Design FMEA versus process FMEA

A DFMEA assumes parts are made to specification and looks at the design itself. A PFMEA assumes the design is correct and examines how manufacturing could create defects. The two link through special characteristics: a design failure with high severity often becomes a flagged characteristic that the process FMEA and control plan must protect. Run the DFMEA first, then carry the critical characteristics forward so neither document hides a gap.

Reading the scores

The risk score here is S times O times D. Sort by score, but treat severity 9 and 10 as mandatory review, because a low occurrence rating at design stage is often optimism rather than evidence. Good DFMEA practice ties each occurrence rating to history, simulation margin or test results. If a line stays high after actions, escalate it to a design change request rather than leaving it as an accepted risk.

Common mistakes

Frequent errors include listing symptoms as causes, rating detection on a test that happens after release, and writing the FMEA once at the end to satisfy a gate. Update it whenever the design, supplier or requirements change. In PLM, link each item to its part number and revision so the FMEA follows engineering change orders and stays traceable in audits.

Frequently Asked Questions

What is the difference between prevention and detection controls?

Prevention controls stop a cause from occurring or make it less likely, for example a design rule or simulation. Detection controls find the cause or failure before the customer does, such as a validation test. They drive occurrence and detection ratings respectively.

When should the DFMEA be started?

As early as concept and requirements are known, and updated through design freeze. Starting late limits the changes you can still afford, so early reviews pay off most.

Does this follow the AIAG-VDA format?

It follows the same logic of function, failure, effect and cause with S, O and D ratings, in a simple table. The full AIAG-VDA form adds structure analysis and action priority, which you can add as columns.

Can I export it for a customer?

Yes. Use the export options to download Excel, CSV or PDF. Check whether your customer requires its own form and copy the columns across.

If your engineering changes and quality records live in different systems, Netray can help connect them so FMEA reviews use current data. This template is free to use on its own.
