IT Change Request Form Template for Plants (CAB Ready)

Most plant outages caused by IT are changes that nobody told the production team about. This form captures the details a change advisory board needs to approve, schedule and, if needed, reverse a change that touches ERP, MES or shop floor systems.

IT Change Request

Company: Acme Precision Ltd, Plant 2 Change ID: CHG-2026-0142 Requested by: A. Patel, Infrastructure Date submitted: 2026-10-20 Title: Upgrade ERP database server firmware and apply OS patches

Classification

ItemValue
Change typeNormal
Risk levelMedium
Planned windowSunday 02:00 to 06:00 local time
Expected downtime90 minutes

Systems Affected

ERP database host DB01, application servers APP01 and APP02, MES interface service, label printing service.

Reason and Benefit

Close vendor security advisories and reduce unplanned reboots. Required before the next ERP release.

Implementation Plan

1. Take snapshot and confirm last good backup. 2. Stop ERP services. 3. Apply firmware and patches. 4. Reboot and verify. 5. Start services and run smoke tests.

Rollback Plan

Revert to snapshot if smoke tests fail or window is 75 percent used. Restore from backup if snapshot revert fails. Decision owner is the on-call infrastructure lead.

Validation and Test Plan

Log in as three roles, post a test receipt, release a test work order, confirm MES transactions and label print.

Production and OT Impact Check

QuestionAnswerOwner
Production schedule reviewed with plant manager?
Shop floor terminals and scanners notified?
Interfaces to MES, PLC gateways or EDI paused?
Recent backup verified and restorable?
Vendor support contact available in the window?

This is a starting point for your internal change process, not an ITIL certification or legal document. Adapt the classification and approval levels to your own policy.

Optional change process review

Send us your current change policy and we will suggest gaps for ERP and MES changes. No obligation.

Optional. The tool and every export work without this. No spam.

How to use the form

Fill in the fields on the left, adjust the sample text, and export to PDF, Word or Excel to attach to your ticket. Describe the systems affected by name, not by category, so the CAB can see which interfaces are in scope. Write the rollback plan before you write the implementation plan, because a change without a reversal path is a gamble. Include a clear trigger for rollback, such as a failed smoke test or the window being 75 percent consumed.

  • One change per form, unless they are truly dependent
  • Name the decision owner for rollback
  • Attach the backup verification result

Why these sections matter

Change type and risk drive who approves. Standard changes are pre-approved and repeatable, normal changes go to the CAB, and emergency changes are fast-tracked with after-the-fact review. The implementation plan should be detailed enough that someone else could run it. Validation steps are tested by business roles, not by IT alone, because a server that boots does not prove that posting a receipt or printing a label works.

The production and OT check

The check table forces the conversation that prevents surprises. Ask whether the plant schedule has a critical shipment in the window, whether handheld scanners and terminals will reconnect cleanly, and whether interfaces to MES, EDI or gateways need to be paused and drained. Keep a record of the answer and the owner. Avoid windows around month-end close, physical inventory counts and big customer deliveries.

Common mistakes

Approving changes without a verified backup, scheduling several changes in the same window so a failure cannot be traced, and closing the ticket without a post-implementation review. Review failed and emergency changes monthly, track change-related incidents, and retire approvals that are never used. Keep the history, because auditors and insurers ask for it.

Frequently Asked Questions

What is a change advisory board?

A CAB is a group of IT, business and operations representatives that reviews and approves risky changes before they happen. In smaller plants it can be a short weekly meeting with the IT manager, ERP lead and a plant representative.

What is the difference between standard, normal and emergency changes?

Standard changes are low-risk and pre-approved. Normal changes need review and approval before scheduling. Emergency changes fix an urgent outage or threat and are approved quickly, then reviewed afterwards.

Do I need a rollback plan for every change?

Yes, even if it is just restore from backup. For some changes there is no clean rollback, and that fact must be stated so the approvers can accept the risk and plan extra testing.

Can this be used for OT changes?

It works as a base, but OT changes often need extra safety review, vendor involvement and production sign-off. Add PLC and HMI specifics and align with your safety and cybersecurity procedures.
