SyteLine Change Request Form Template
Uncontrolled changes are the reason a Tuesday setting breaks Friday's shipments. This form records the why, what, who is affected, how it was tested and how to undo it, before anyone touches the production SyteLine system.

Change Request CR-2026-041: Add inspection hold step to receiving for AS9100 parts
Requestor: Quality manager Date: 2026-10-20 Type: Customization (form, IDO, event handler) Risk: Medium
Reason and description
Reason: Aerospace parts must not move to stock before incoming inspection is recorded. The current process relies on a manual note.
Description: Place received lots for flagged items into an inspection location and release only after a quality user posts the result.
Impact analysis
Receiving, Inventory, Quality. Affects about 12 users on two shifts. No change to financial postings.
Test and rollback
Test plan: Test in the TEST environment with three flagged items: pass, fail and partial. Confirm location, lot status and ledger entries.
Rollback plan: Restore the previous form customization package and revert item flags from the saved export.
Implementation window: Saturday 07:00 to 09:00, outside production hours

Approvals
Scale the form to your process: low risk changes can use a shortened path while high risk or financial changes need extra approvers. Align with your own ISO 9001, AS9100 or IT control requirements.
Review your customizations
Optional assisted inventory of customizations and their upgrade impact.
Optional. The tool and every export work without this. No spam.
How to use the form
Complete one request per change. State the reason in business terms, describe the change precisely enough that another administrator could repeat it, and list who and what it affects. Set a risk level honestly, since it decides who must approve. Attach screenshots, scripts or exported settings. After approval, implement within the planned window and complete the readiness checks.
- Number every request for traceability.
- Always test in a non-production site first.
- Take an export before changing settings.
- Record who verified after go-live.
Impact analysis questions
Ask which forms, reports, IDOs, event handlers and integrations touch the object being changed. Check scheduled jobs, label or document outputs, EDI maps and BI extracts that read the same fields. Consider upgrade impact too: customizations have to be carried through each patch or version change, so each new one adds to your upgrade testing scope.
Risk levels and approvals
Low risk changes affect one user group, are easy to undo and do not touch money or compliance records. Medium risk changes alter shared processes or integrations. High risk changes involve financial postings, inventory valuation, traceability data or security. Define who approves each level in your own policy, and keep the signed forms for auditors.
Common mistakes
Testing only the happy path, forgetting the rollback, changing production directly to fix an urgent issue and never documenting it, or approving without reading the impact. Emergency changes should still be written up afterwards, with a retrospective review in the next change meeting.
Frequently Asked Questions
Do configuration changes need change control?
Yes, when they affect shared behaviour such as costing methods, numbering, security, defaults or planning parameters. A short version of the form is fine for low risk settings, but the record should exist.
What counts as an emergency change?
A fix needed to restore a stopped process, such as shipping or receiving. Make the minimum fix, document it as an emergency, and complete the full form with a review after the event.
Where should we store approved forms?
In a controlled, searchable location such as your ticketing system or document management tool, linked to the change number. Auditors usually ask for a sample of changes and their evidence.
How is this different from a project change order?
A project change order alters scope, cost or timeline of an implementation. This form controls technical and process changes in a live system. Large projects often need both.
