AS9102 Form 1 Part Number Accountability Template

AS9102 defines a first article inspection report in three forms. Form 1 is the cover record that says which part, drawing revision and lot the FAI covers, why it was triggered and who signed it. This template builds that page in your browser and exports it to PDF, Word or Excel.

Structure follows the Form 1 intent of AS9102 (part identification, FAI scope, approval). Always confirm field wording against the AS9102 revision and customer-specific requirements that apply to your contract. This is a working template, not the controlled standard form.

Pre-fill FAI packages from your ERP

Want part, revision and lot data pulled into your FAI forms automatically? Ask for a short review of your setup.

Optional. The tool and every export work without this. No spam.

How to use the template

Enter the part, drawing and revision exactly as they appear on the released drawing and the purchase order. Select the FAI type and the trigger, then add the baseline report number if you are doing a partial FAI. Export the document and attach Form 2 and Form 3 plus the ballooned drawing. Everything stays in your browser, nothing is sent to a server, so you can use real part data.

  • Copy part and drawing revision from the released drawing, not from memory
  • One FAI report number per part number and lot
  • Name the trigger in plain words so the reviewer sees the scope at a glance
  • Keep the export with the lot traveler

What AS9102 asks of Form 1

AS9102 requires an FAI when a part is new, when the design changes, when the process or source changes, or when production has lapsed for a defined period (the standard cites two years unless the customer specifies otherwise). Form 1 records the identification of the part, the drawing and revision, the FAI type and the signatures. A full FAI covers every characteristic; a partial FAI is limited to what changed and must reference the baseline report.

Worked example

A machine shop receives a revision change on a bracket that moves a bore location and changes a plating callout. It selects Partial FAI, reason Engineering change, cites the earlier full FAI as baseline, and inspects the bore, the surrounding dimensions that could be affected and the new plating process. Form 1 shows the scope, Form 3 lists only the affected characteristics, and Form 2 lists the new plating certificate.

Common mistakes and ERP notes

The usual mistakes are a drawing revision that does not match the purchase order, a missing baseline reference on a partial FAI, and a report number that is reused across lots. In an ERP such as SAP QM, Infor SyteLine, Epicor or Oracle, store the FAI report number against the lot or serial and make the inspection plan check that a current FAI exists before first shipment.

Frequently Asked Questions

Is this the official AS9102 form?

No. AS9102 is published by SAE International under the IAQG and the standard text is copyrighted. This tool follows the structure and intent of Form 1 so you can produce a clean record, and you should map it to the form wording your customer and registrar expect.

When do I need a partial FAI instead of a full one?

A partial FAI applies when only some characteristics are affected by a change, such as a revised dimension or a changed process step. You need a baseline FAI to reference, and you must justify why the unaffected characteristics need no reinspection. A new part or a new source normally needs a full FAI.

Who signs Form 1?

The person who performed or compiled the FAI and an authorized quality representative sign it. If your customer requires review or approval of the FAI package, their delegate signs after your release. Check the purchase order and any flowed-down quality clauses.

Can I export it to Excel?

Yes. The document exports to PDF, Word and Excel from the page. Many suppliers keep Form 1 in a spreadsheet alongside Form 3 so reviewers can see both in one workbook.

If you are copying part, revision and lot data into Form 1 by hand, we can pull it from your ERP and pre-fill the FAI package on your own servers.
