Field structure follows the purpose of AS9102 Form 2. This is a working template, not a reproduction of the standard. Always confirm the required layout and any customer-specific additions against your current copy of AS9102 and the purchase order.

Pull FAI data from your ERP

Optional: we can review how material lots and process approvals could feed your FAI package automatically.

Optional. The tool and every export work without this. No spam.

How to use this template

Enter the part identification at the top, then record each raw material, purchased component and special process used on the first article. Add the certificate or lot references that support each line. Use the table rows for extra materials or processes, then export to PDF for the FAI package or to Word if the customer wants edits. Keep the certificates themselves attached to the package as evidence.

  • One row per material or purchased part
  • One row per special process, including sub-tier suppliers
  • Reference the cert number, not just the supplier name
  • Record functional test acceptance criteria and the result

What Form 2 is meant to prove

Form 1 identifies the part and Form 3 accounts for every drawing characteristic. Form 2 sits between them and answers a different question: did the right material go through the right approved processes, and did the part work when tested? An auditor or customer reviewer should be able to trace each line to a certificate and an approved source without asking you for more paperwork.

Special process approval details

For processes such as anodize, heat treat, plating, welding and NDT, record the specification and the supplier approval status. Many primes require Nadcap accreditation or a direct customer approval for the process in question, so capture the scope and expiry date you verified. If the supplier's approval lapsed before the work was done, that is a nonconformance to raise before the report is released.

Common mistakes

Most Form 2 rejections come from missing traceability rather than bad parts. Typical problems include a certificate that names a different specification revision, a process line with no supplier approval evidence, a functional test with no stated acceptance criteria, and sub-tier processes left off because the prime supplier did not mention them. Check each row against the purchase order flow-down before you sign.

Frequently Asked Questions

Is this the official AS9102 form?

No. It is a working template that follows the structure and purpose of Form 2. AS9102 is published by the IAQG and distributed through SAE and national bodies, so confirm the required format and your customer's requirements against the current standard.

Do I need Form 2 for every first article?

Form 2 applies when the part uses designed or specified materials, special processes or functional testing. If a part has none of these, the form can be marked not applicable, but check with your customer because some require the form regardless.

Can I list several materials and processes?

Yes. Add as many rows as you need. Each material, purchased part, special process and test should have its own line with its own certificate reference so reviewers can trace it.

Does the output include my data on the server?

The template runs in your browser and exports directly, so what you type stays on your machine unless you choose to contact us.

If your ERP already holds material lots, supplier approvals and routing steps, Netray can help pull them into this form automatically instead of retyping them.
