CAPA Action Tracker Worksheet for Excel

A CAPA that sits at the root cause stage for four months is a finding waiting to happen. This tracker lists every action with owner, due date and ageing so the quality manager can see what is late before the auditor does.

Remove closed CAPAs from the active view after the effectiveness check passes, and keep them in an archive. A common target is closing most CAPAs within 30 to 90 days, but your quality system or customer sets the real limit.

Keep the CAPA list current from your ERP

Optional: we can connect your ERP or quality system so open CAPAs and ageing refresh without manual updates.

Optional. The tool and every export work without this. No spam.

How to use the tracker

Add one row per CAPA with its source, owner and due date. Update the status as the work moves through containment, root cause, action and effectiveness check. At each weekly review, refresh the days open and the days past due columns. The overdue flag turns to 1 whenever days past due is above zero, and the summary shows the share of CAPAs that are overdue and the average lateness. Sort by days past due in Excel to build the agenda for the review meeting.

  • One named owner per CAPA
  • Review weekly with the same agenda
  • Update days open at each review
  • Archive closed items to keep the view short

What the metrics tell you

A rising overdue share tells you that the CAPA load exceeds capacity or that actions are being opened for issues that do not justify one. The oldest CAPA and the average age show whether work stalls at a stage, usually root cause analysis. Count the status column too: a pile of CAPAs in effectiveness check means the closure step is neglected. Management review under ISO 9001 and AS9100 expects trends on corrective action performance, and this data supports it directly.

Worked example

In the sample data, three of five CAPAs are overdue, so the overdue share is 60%. The oldest is 98 days open, and the average days past due is 20.8 across all five. The quality manager sees that two of the late items belong to the same owner, and that the oldest is a training record issue stuck in the action stage. The response is to rebalance ownership and set a firm date, not simply to chase.

Common mistakes

Keeping the tracker separate from the quality records leads to two truths. Others include allowing the owner to be a department rather than a person, closing a CAPA when the action is done instead of when it is proven effective, and opening a CAPA for every minor nonconformance. Use criteria for opening one, such as repeat events, customer impact or audit findings, and keep the rest in the NCR log.

Frequently Asked Questions

How many CAPAs should be open at once?

There is no fixed number. What matters is that every one has an owner, a date and visible progress. A long list of stale items is a bigger warning sign than a long list of active ones.

What counts as overdue?

Any CAPA past its committed due date for the current stage or for the whole action. Agree a rule in your procedure, and record approved extensions with a reason instead of silently moving dates.

Can I use this for preventive actions too?

Yes. Add a source such as risk assessment and track preventive actions in the same list, since they share the same owner, date and status structure.

Does the tracker replace my quality management system?

No. It is a working list for reviews. If you must keep controlled records, store the full CAPA documents in your quality system and use this as the summary view.

Netray can feed this tracker from your ERP or quality system automatically, so days open and overdue status are always current.
