MRB Material Review Board Disposition Form Template

A Material Review Board form is the record that shows who decided what to do with nonconforming material, and why. This template gives you a clean one-page MRB disposition document you can complete at the inspection bench and send to the board.

Material Review Board Disposition

Company: Acme Precision Ltd MRB number: MRB-2026-0147 Date: 2026-10-20 Part number and revision: PN-48213 Rev C Lot or serial numbers: Lot 26-3318, 40 pcs Source: Receiving inspection Quantity nonconforming: 6

Nonconformance

Requirement: Bore diameter 12.700 +0.025 / -0.000 mm (Dim 7)

Finding: Six of 40 pieces measured 12.742 to 12.755 mm on the bore, above the upper limit. Remaining pieces conform.

Disposition and Rationale

Disposition: Return to vendor

Oversize bore reduces press fit retention. Not acceptable as is. Supplier to replace and provide corrective action.

Containment

Lot quarantined in MRB cage. Open stock of lot 26-3318 sorted. Previous two lots checked, no issues found.

Use as is and repair dispositions usually need customer approval on aerospace, defense and automotive contracts. Check your contract and quality manual. Starting point only, adapt it to your quality system.

Automate MRB dispositions from your ERP

Optional: see how MRB decisions can post inventory transactions and update supplier scorecards automatically.

Optional. The tool and every export work without this. No spam.

How to use the MRB form

Fill in the identification block first: MRB number, part number with revision, lot or serial numbers and the source of the finding. Then state the drawing requirement and the actual finding with measured values, not just the word defective. Pick the disposition the board agreed, write the rationale in plain language, and list containment. Export to PDF or Word and attach it to the nonconformance record in your ERP or QMS. Keep one form per lot and per failure mode so the audit trail stays readable.

What the board decides

The usual dispositions are use as is, rework to print, repair, scrap, return to vendor, and sort and accept the conforming pieces. Use as is and repair are special because they accept a departure from the drawing, so they need engineering justification and often customer approval. Rework brings the part back to print using an approved process. Scrap and return to vendor close the loop through inventory transactions and, for supplier causes, a corrective action request.

  • Quality engineer: confirms the finding and the requirement
  • Design engineer: judges form, fit and function risk
  • Procurement or supplier quality: owns return and recovery
  • Customer: approves use as is or repair when contract requires

Writing a defensible rationale

An auditor reads the rationale to see whether the decision followed from the risk. Name the characteristic, say how far out of tolerance it is, and say what function it affects. Example: an oversize bore on a press fit reduces retention, so use as is is rejected. Avoid phrases like engineering says OK. If a use as is is chosen, reference the analysis, test or drawing note that supports it and record the approver by name and date.

Common mistakes and ERP notes

Common failures are mixing several defects in one form, leaving quantity blank so inventory cannot be corrected, and closing the MRB without a corrective action on a supplier cause. In ERP systems, tie the MRB number to the quarantine or nonconforming location, then post the scrap, return or rework transaction so the stock record matches the physical result. Record the MRB number on the receiving document so the supplier scorecard picks up the rejection.

Frequently Asked Questions

Who sits on a material review board?

At minimum, quality and engineering. Procurement or supplier quality joins for purchased material, and the customer representative is involved when the contract requires approval of use as is or repair dispositions. Your quality manual defines the exact membership.

Is an MRB the same as an NCR?

No. The nonconformance report records the defect and containment. The MRB form records the board's decision on disposition. Many companies combine them on one form for minor issues, but separate records are common for use as is and repair decisions.

Can I use as is without customer approval?

Only if your contract and quality system allow it. Many aerospace, defense and automotive contracts require customer approval for use as is or repair of a nonconformance against the drawing. Check the contract terms before the board decides.

How long should MRB records be kept?

Follow your quality system and customer flow downs. Many aerospace and defense programs require several years or the life of the program, and some longer. Set the retention period in your records procedure and apply it consistently.

If your MRB forms live in spreadsheets and the ERP stock record is updated by hand afterwards, Netray can help connect the two so dispositions post and report automatically.
