Food Safety Corrective Action Report Template

When a CCP goes outside its critical limit, the corrective action record is the evidence that you contained product, fixed the cause and prevented a repeat. This template gives you a structured form that exports to PDF or Word.

Corrective Action Report

Site: Riverbend Foods Ltd, Plant 2 Report number: CAR-2026-0187 Date of deviation: 2026-10-19

Deviation details

Process step: CCP 1 Cook, spiral oven line 2 Critical limit: Minimum core temperature 74 C Observed value: 73.1 C at 09:05 Affected lots: L2911, 14 pallets, 09:05 to 09:40 production

Product disposition

Decision: Reprocess

Basis for decision: attach hazard evaluation, test results or reprocess records.

Root cause

Belt speed setpoint was changed during a product changeover and not restored to the validated value.

Corrective action

Line stopped, belt speed restored to 4.2 m/min, probe thermometer verified, product held and re-cooked under supervision.

Preventive action

Add belt speed check to changeover checklist and lock recipe parameters to authorized users.

This template is a starting point and not legal or regulatory advice. Align the wording with your HACCP plan, your certification scheme and local regulation, and retain the record for the period they require.

Link corrective actions to ERP lots

Optional review of how holds, lots and CAR numbers can be tied together in your ERP.

Optional. The tool and every export work without this. No spam.

How to fill it in

Start with the facts: the CCP, the critical limit, the observed value and the exact lots made since the last good check. Choose the product disposition after a documented evaluation. Then write the root cause as a specific mechanism, not a person. Corrective action covers what you did now; preventive action covers what changes so it cannot happen again. Print the PDF, sign it, and file it with the monitoring log.

  • Bracket affected product between the last good check and the next good check
  • Attach the evaluation, test or reprocess evidence to the report
  • Keep corrective and preventive actions as separate statements

What regulators and auditors look for

HACCP principle 5 requires corrective actions to be established for deviations. The FDA preventive controls rule for human food expects corrective actions, and where relevant corrections, to identify and correct the problem, reduce the likelihood of recurrence, evaluate the safety of affected food, and prevent unsafe food entering commerce. Certification schemes ask for the same logic. A report that shows each of these steps is straightforward to defend.

Common mistakes

The most frequent weakness is a vague root cause such as operator error with no system change behind it. Second is releasing held product with no evaluation documented. Third is closing the report without checking whether the fix worked. The verification table in this template forces an effectiveness check by date and method so the report is not closed on intention alone.

Frequently Asked Questions

What is the difference between a correction and a corrective action?

A correction is the immediate fix to the situation, such as holding and re-cooking product. A corrective action addresses the cause so the problem does not recur, such as locking a recipe parameter. Strong reports record both.

Who must approve product disposition?

Your procedure decides, but it is normally the food safety team leader or QA manager, with a qualified person where regulation requires it. Disposition should rest on documented evaluation, not production pressure.

How long should I keep these reports?

At least the period required by your regulator, customer and certification scheme. Under FDA preventive controls rules records are generally kept for two years, and shelf life can extend it. Check your own requirement.

Do I need a report for every deviation?

Yes for every deviation from a critical limit. Minor operating target misses that stay within critical limits may be handled through routine quality records. Define the threshold in your procedure.

If corrective actions are tracked in email or spreadsheets, we can help connect them to lot, work order and inventory hold records in your ERP.
