Packaging Artwork Proof Approval Form Template
Artwork errors are the most expensive mistake in packaging because they are discovered after plates, cylinders or dies are made and thousands of units are printed. This form turns the proof review into a single signed record. Fill in the fields, tick the checklist, and export to PDF or Word.

Approvals
Starting point only. Add your customer's own artwork control steps and keep the signed copy with the job record. Barcode grade should be verified with a verifier on the printed sample, not only on screen.
Link artwork approval to your ERP job
Optional: a short review of how to block job release until artwork sign-off is recorded in your ERP.
Optional. The tool and every export work without this. No spam.
How to use the form
Complete the job header and artwork version first, because the version string is what protects you later when two PDFs circulate. Describe what changed against the previous proof so the reviewer knows where to look. Print or export the document, then have the customer, quality or regulatory reviewer and production release sign. Store the signed form with the job ticket or attach it to the job in your ERP.
What a good proof review covers
A proof review is more than a visual check. Compare the die line against the approved structure, confirm every language version, and verify that mandatory copy such as allergens, net quantity and recycling marks is present and legible. Confirm barcode number and quiet zones, and note any spot colour or special effect that a PDF cannot show accurately.
- Always compare against the previous approved version, not only against the brief
- Check text in every language and every pack size in the family
- Soft proofs cannot confirm colour, so state that on the form
- Record who approved, on which date and which version
Common mistakes
The usual failures are approving a file that was not the one sent to plate, skipping a language variant, and changing a number such as net weight on one panel but not another. Another is accepting a PDF proof for a colour-critical brand colour. Use the Proof type field honestly and require a press proof or wet proof where colour matters.
- Version name on the PDF does not match the form
- Barcode checked on screen but never verified when printed
- Verbal approvals with no signature
Frequently Asked Questions
Who should sign off packaging artwork?
Usually the customer or brand owner for design and copy, the converter's prepress for technical accuracy, and a quality or regulatory person where legal copy applies. Production release comes last, once the signed form confirms the version going to plate.
Is a PDF proof enough to approve colour?
No. A PDF on a screen cannot reproduce spot colours, varnishes or foils reliably. Use a calibrated digital contract proof or a press proof for colour-critical work, and state the proof type on the form so the limits are clear.
How long should approved artwork records be kept?
Keep them at least as long as the product shelf life plus your customer or regulatory retention requirement. Many food and pharma customers ask for several years. Check the quality agreement for the exact period.
Can I use this for labels as well as cartons?
Yes. Change the item description and the checklist rows to suit labels, for example adding unwind direction and liner details. The core sequence of version, change summary, checks and signatures stays the same.
If your artwork approvals live in email threads, we can help connect proof status to the job in your ERP so release is blocked until sign-off is recorded.
