SyteLine User Access Request Form Template

Most SyteLine audit findings trace back to user access that nobody can explain. This form gives you one page per request, from requestor to administrator, with a segregation of duties review before anything is granted.

Automate your SyteLine access review

We can build a read-only report of users, groups and sites that flags likely conflicts for your reviewers.

Optional. The tool and every export work without this. No spam.

How to use the form

Fill in the requestor details, then list the authorization groups and the specific forms needed. In SyteLine, access is normally granted through user records linked to authorization groups and, where used, site and entity assignments. Writing the request in those terms stops vague asks like 'same access as Sam'. Export to PDF or Word, route it for signatures, and file it with the ticket that the administrator uses to create or change the user.

  • Use one form per person per change.
  • Name the sites, not just the company.
  • State an end date for contractors and temporary cover.
  • Attach the signed copy to the change ticket.

The segregation of duties block

The review table lists classic conflicts in a manufacturing ERP: buying and receiving, vendor maintenance and payment, inventory adjustment and count approval, cost maintenance and ledger posting, and user administration. A conflict is not an automatic refusal. It means a named compensating control must exist, such as a manager review of adjustment reports or a monthly exception report. Add rows for conflicts that matter in your business.

Terminations and periodic review

Use the Terminate user type on the day someone leaves, and disable the account rather than deleting it so that transaction history keeps a valid user reference. Run a full access review at least twice a year: export the list of users and groups, send it to line managers, and require a signed confirmation. The same form can be reused as the record of any change that results.

Common mistakes

The usual failures are shared logins for shop floor terminals, access copied wholesale from a colleague, administrators approving their own requests, and no end date on consultants. Also check integrations: service accounts used by APIs or reporting tools need the same discipline and should be listed on a separate register with an owner.

Frequently Asked Questions

Does SyteLine need a separate request for each site?

List every site or entity the person needs on one form. The administrator then assigns the matching access per site. If different sites have different data owners, add a signature line for each owner so each approves their own data.

Who should approve access?

At minimum the line manager and the owner of the data being accessed, such as the controller for finance or the materials manager for inventory. The administrator should execute but not be the only approver. Internal audit or IT control signs where conflicts are found.

Is this form enough for SOX or ISO audits?

It is a good evidence record, but auditors also want periodic access reviews, termination timeliness and administrator activity logs. Use the form as one control within your own documented process and check requirements with your auditor.

Can I use it for non-SyteLine systems?

Yes. Rename the product references and the authorization group wording. The structure of requestor, access, justification, conflict review and approvals works for any ERP or business application.

If you want user and group listings pulled from SyteLine and compared against this kind of policy automatically, we can build a read-only review report on your data.
