This is an internal investigation template, not the statutory form. The Factories Act and your state rules prescribe notice timelines and formats for accidents and dangerous occurrences, and the labour codes may change them. Confirm obligations with your EHS or legal adviser. It is a starting point, not legal advice.

Connect incident data to production and maintenance data

Optional: discuss analysing incidents against machine and shift data from your ERP.

Optional. The tool and every export work without this. No spam.

How to use the template

Fill the fields while facts are fresh, ideally within the same shift. Describe what happened in plain words, avoiding blame. Record the injury and treatment, then spend time on the root cause, because the real fix is usually a missing guard, an unclear procedure or a bypassed interlock rather than operator error. Export to PDF or Word, add photos if needed, and obtain signatures. Edit the action table to match your findings.

  • Preserve the scene and the machine state before cleanup
  • Interview the worker and witnesses separately
  • Ask why five times to reach the system cause
  • Close each action with an owner and due date

Statutory side

Indian factories are required to give notice of certain accidents, usually those causing death or injury beyond a defined threshold, and of listed dangerous occurrences, within time limits set by the Factories Act and state rules. ESIC rules also require reporting for insured workers. The checklist in this template is a reminder only. Use the prescribed forms and timelines applicable in your state.

Common mistakes

Plants often stop at the proximate cause, such as worker carelessness, and repeat the accident a few months later. Others do not record near misses, losing the best early warning. A third mistake is treating contract workers as someone else's problem, when the principal employer carries duties for safety at the site. Include every worker in the report process.

Frequently Asked Questions

Is this the statutory accident form?

No. It is an internal investigation and record template. Statutory notices must be made in the prescribed form and time under the Factories Act, state rules, and ESIC where applicable.

Should near misses be reported?

Yes. Near misses show the same hazards without the injury. Use the same template, select Near miss, and track actions to closure.

Who should sign the report?

At least the person reporting, the department head, the EHS officer and the factory manager. Add the contractor representative when a contract worker is involved.

Can I edit the actions table?

Yes. The action rows are examples. Replace them with actions that address your root cause, with owners and dates, in the exported document.

If you want incident, maintenance and production data connected so patterns across machines show up automatically, we can discuss that. The template stands on its own.
