How to complete the NCR
State the requirement and the actual result in measurable terms such as center wavelength, transmission or wavefront error. Record quantity affected and where the defect was detected. Contain suspect stock before debating the cause. Choose a disposition, route for signature, and link the NCR number to the receipt, work order or lot in your ERP.

Typical photonics nonconformances
Frequent causes include coating shifts, scratch and dig excursions, contamination, alignment drift, adhesive voids and laser power outside tolerance. The measurement record helps separate a supplier issue from an internal process issue and supports a supplier corrective action request when needed.
Closing the loop
An NCR that ends at disposition solves one lot. Link it to a corrective action when the cause is systemic, and review recurring defect types monthly. Many ERPs have a quality notification or nonconformance object that can hold these records so trending is automatic.

Who approves the disposition?
That depends on your procedure. Many organizations use a material review board with quality and engineering, and customer approval is required for use as is or repair on some contracts.
Is this form AS9100 compliant?
It contains the elements usually expected: identification, description, containment, disposition and approvals. Compliance depends on how your quality system uses it, so check against your own procedures.
What is the difference between an NCR and a CAPA?
An NCR records and dispositions a specific nonconformance. A CAPA addresses the underlying cause to prevent recurrence. One CAPA can cover several NCRs.
Can I use it for supplier returns?
