How to use the template

Fill in the organisation, vendor and repair order number, then the part identity: part number, serial number, time since new and time since overhaul. Describe the removal reason in a way that tells the shop what to look for. Choose the scope and the release document you need. Export the page and attach it to the purchase order.

What the vendor needs

A vendor can only quote accurately with the part number, serial, history and a clear fault description. Without them the shop quotes a worst case or starts with an evaluation. Stating the authorisation limit up front prevents surprise invoices and tells the shop when to stop and call.

  • Part and serial numbers exactly as marked on the unit
  • Time and cycle history from your records
  • Reported defect and troubleshooting performed
  • Scope, release type and spend limit

Release documents

Specify the release you need. The FAA Form 8130-3 and EASA Form 1 are the common authorised release certificates for components, and the one you need depends on the aircraft registry, bilateral agreements and your approval. If in doubt, confirm with your quality department before the unit ships.

Common mistakes

Sending a unit with no fault description, forgetting to state the release required and not setting a spend limit are the common faults. Also check that sub-components removed during repair are traceable, since lost records on life-limited items can make the whole unit unusable.

Frequently Asked Questions

Do I need a separate request per unit?

Yes, for traceability each serialised unit should have its own request line or form, even when several units go to the same vendor in one shipment.

What should the removal reason include?

The crew or maintenance report, the troubleshooting done and the test result. Be specific about symptoms and conditions. Vague wording like unit faulty leads to a long evaluation.

Why state an authorisation limit?

It stops the shop proceeding with expensive work before you have approved the quote. If the cost would exceed the limit, they must stop and send an estimate first.

Is this form a contract?

No. It supports your purchase order and does not replace its terms. Have your commercial and legal teams review the conditions before relying on them.
