Cost of ISO 13485 Certification: Free Estimator
The cost of ISO 13485 certification splits into three buckets: what the registrar charges for audit days, what a consultant charges to build the system, and the internal time your own people spend. Enter your quotes and the estimator shows year one and the full three-year cycle.
Your numbers
Document review and readiness. Your registrar sets this from headcount, sites and risk class.
On-site implementation audit. Small firms of 20 to 50 people commonly see 2 to 5 days.
Get written quotes from at least three accredited certification bodies.
0 if you build the QMS yourself.
Writing procedures, training, internal audit, management review, records set-up.
Your results
Planning estimate only. Registrar audit days follow the certification body's own method for the applicable accreditation rules, so request formal quotes. Surveillance and recertification ratios are typical assumptions, not guarantees.

How to use the estimator
Start with the registrar quote, because audit days are the least negotiable item. Enter the Stage 1 and Stage 2 days from the quote, the day rate and travel. Then set consultant days (use 0 if you are writing the system yourself) and the hours your own staff will spend. Training and tools covers an internal auditor course, document control software and any calibration you must catch up on. The year one total is the number to put in front of management; the cycle total is the honest number for budgeting.
- Get at least three written quotes from accredited certification bodies
- Ask whether MDSAP or other scheme audits can be combined to share audit days
- Count your own people's time, it is usually the biggest hidden item
- Use the chart to see how much each extra consultant day moves year one
What drives audit days and fees
Certification bodies set audit duration from the number of effective personnel, the number of sites, the complexity of processes and the risk of your device types. Sterile, implantable and software-driven devices usually draw more days than simple non-sterile products. Design and development activities, sterilization, and outsourced processes add scope. Fees then scale with the day rate, travel and any annual management fee. After the first certificate, surveillance audits are roughly a third of the initial on-site effort each year, and recertification at year three is closer to two thirds.
Worked example
A 35-person contract manufacturer with one site books 4.5 audit days at $1,400 plus $150 travel, so the registrar total is about $7,500 with fees. A consultant for 12 days at $1,500 is $18,000. Internal effort of 300 hours at $55 adds $16,500 and training and tools another $3,500. Year one lands near $45,500. Cutting consultant days to 6 and shifting a few more hours inside saves less than people expect, because internal hours cost real money too. That is why the estimator prices both sides.
Common mistakes
Teams budget only the audit and then discover the build cost. Others hire a consultant to write a thick manual nobody uses, then pay again to fix it after a major nonconformity. Watch for these.
- Ignoring remediation after Stage 1 findings that trigger an extra visit
- Forgetting the internal audit and management review that must happen before Stage 2
- Not budgeting software validation for QMS tools under the software validation requirements
- Assuming surveillance is free once certified
Frequently Asked Questions
How much does ISO 13485 certification cost for a small company?
A small single-site company often spends from the low tens of thousands of dollars in year one when registrar fees, some consulting and internal time are all counted. The registrar portion alone is commonly a few thousand to under ten thousand. Use your own quotes in the estimator to get a real figure.
Is the certificate a one-time cost?
No. The certificate cycle is three years. You pay for annual surveillance audits and a recertification audit at the end of the cycle, plus the internal effort to keep the system running. The estimator shows the full cycle total.
Do I need a consultant?
No. Many teams with a quality manager who knows the standard build the system themselves. A consultant mainly saves time and reduces the risk of a failed Stage 2. It trades money for calendar time, so price both options.
Does ISO 13485 certification alone allow me to sell in the US or EU?
No. The certificate shows your QMS meets the standard. Market access needs device registration, 510(k) or MDR conformity assessment and similar steps, with their own costs. MDSAP can add audit scope for several countries.
